Fast magnetic resonance diffusion‐weighted imaging of acute human stroke

Abstract: Rapid MRI of the molecular diffusion of water demonstrated cerebral infarcts in 32 patients. We studied these patients at various times following the onset of ischemic symptoms and found that diffusion-weighted imaging revealed the infarcts sooner than conventional T2-weighted spin-echo imaging did; four hyperacute infarcts were shown only by diffusion-weighted imaging. Acute infarcts had lower apparent diffusion coefficients (ADCs) than noninfarcted regions did. This relative difference in ADC reached a nadir… Show more

“…So far, diffusion weighted imaging has not been proven to be effective as a prognostic tool in NE. 14 This is surprising, as diffusion weighted imaging in the adult brain 15 and experimental models 16 of stroke has revolutionised the possibility of early diagnosis and intervention, often revealing image abnormalities on water diffusion maps well before CT or conventional MR. 31 P and 1 H MR spectroscopy (giving quantitative information on brain energetics, intracellular pH (pH i ) and lactate) can give early prognostic information on brain injury severity. However, for logistical reasons it is almost impossible to transport sick infants with NE to an MR system and perform an MR study within six hours of birth.…”

“…For example, DW-MRI allows the detection of cerebral ischemia at a very early stage (1,2). Following studies on animal models of stroke (1-10), DW-MRI has been successfully applied in the clinic (11)(12)(13)(14)(15). Nevertheless, in vivo DW-MRI remains technically challenging because of motion problems, since DW-MRI methods are typically sensitized to diffusion displacements of only a few micrometers.…”
