Fatal, complete splenic infarction and hepatic infection due to disseminated Trichosporon beigelii infection

Abstract: We report on a 56-year-old woman with acute lymphocytic leukemia who presented with right upper quadrant pain, fever, nausea, and vomiting. Laboratory studies confirmed fungemia with Trichosporum beigelii, and contrast-enhanced computed tomography of the abdomen demonstrated numerous low-attenuation liver lesions and a hypodense spleen with capsular enhancement suggestive of complete splenic infarction. Subsequent splenectomy confirmed that the spleen was completely infarcted and infiltrated with Trichosporum.… Show more

“…Optimal therapy for invasive trichosporonosis is undefined and sparse data are available. Several antifungal agents, such as amphotericin B, fluconazole, and voriconazole, have been used either as monotherapy or in combination (3–14, 17–21, 23–27). Echinocandins do not seem to have activity against these yeasts and reports of breakthrough infections in patients receiving caspofungin or micafungin have emerged (7, 8, 18, 30, 31, 33, 34).…”

“…Trichosporon species are basiodiomycetous yeasts that predominantly cause superficial skin infections and scalp or pubic white piedra, but that can also cause invasive disease in immunocompromised individuals, especially those with hematological malignancies, profound neutropenia, or previous hematologic transplant (1–15). Trichosporonosis appears to be less common in other immunosuppressed states such as solid organ transplant recipients or in acquired immunodeficiency syndrome (16–22).…”

“…commonly seen in sickle cell disease, but there are also reports in systemic lupus erythematosus in about 5% [9,10] , after bone marrow transplantation [11] and Rothmann-Makai syndrome (lipogranulomatosis subcutanea) [12] . Viscomi et al [13] described massive splenic infarction due to Trichosporon beigelii infiltration of the spleen. Another interesting complete splenic infarction was described in a patient with paroxysmal nocturnal hemoglobinuria due to thrombi in splenic veins by Tsatalas et al [14] .…”
