Fatal methadone and heroin overdoses: time trends in England and Wales.

Abstract: Study objective-Although the total number ofselfpoisonings in England and Wales has dropped by 32%, the number involving methadone and/or heroin rose by 900% in 1974-92. Because ofconcern about the role of methadone in this increase, the part played by methadone and heroin in poisoning deaths in England and Wales in 1974-92 was investigated. “…However, as Neeleman & Farrell observed [20], the reported increased deaths involving methadone appeared to be merely tracking the increased extent of the heroin problem in society as measured by increase in deaths from heroin itself [20]. Furthermore, evidence was emerging that many of these methadone deaths were as a result of diverted supplies of prescribed methadone, with the deaths being of individuals who were not the index patients to whom the drug had been prescribed.…”

“…3 With this increase in methadone prescribing, an increasing number of deaths associated with methadone has been recorded in England and Wales. 4 Overall in western Europe, there was a sixfold increase in methadone dispensed between 1981 and 1994. More methadone for each head of population is prescribed in Denmark, Switzerland, and the Netherlands, but the other western European countries prescribe less.…”

“…The bene® ts of methadone maintenance therapy, including a reduction in overall mortality, have been well established (Preston 1996, Neeleman andFarrell 1997). Indeed, evidence from the CDPS in Lothian indicated a reduction in intravenous drug use and needle sharing, a stabilisation of drug intake and a reduction in criminal behaviour with consequent social stability among its clients (Greenwood 1990).…”
