The study objective is to evaluate the results of our surgical technique for children with congenital adrenal hyperplasia and ambiguous genitalia at the University Hospital of Heidelberg, Department of Paediatric Surgery. The records of 19 patients with congenital adrenal hyperplasia treated between 1972 and 2004 were reviewed with respect to age at surgery, operative procedures and outcome. We describe the recession clitoroplasty technique currently used in our hospital and highlight the importance of short and long-term follow-up results with respect to appearance, position and size of the clitoris and quality of the vagina. One-stage recession clitoroplasty and vaginoplasty gives very satisfactory cosmetic and functional results, with few complications and a reduced need for secondary surgical interventions. The results of this study support the assumption that total correction can be achieved through a single-stage operation, performed in infancy.

Aim The aim of the study was to review the current approach to manage ambiguous genitalia caused by CAH.Methods This was a retrospective study of 30 patients with CAH and ambiguous genitalia treated over 10 years. Age at presentation, degree of verilization, preoperative diagnostic studies, operative technique, blood loss, outcome, and follow-up period were the studied variables. ResultsThe older 19 patients underwent single-stage feminizing genitoplasty and the younger 11 patients underwent a multiple-stage procedure. All patients had undergone clitoroplasty, clitoral recession, or resection. Postoperative complications documented in 23% of the patients included atrophy of the clitoris, flaps complication, and vaginal stenosis. The anatomic and cosmetic outcomes were considered good or satisfactory in 70% and 20% of cases, respectively, and poor in 10% of cases.Conclusion One-stage or multiple-stage feminizing genitoplasty gives a high percentage of satisfactory cosmetic outcomes. Abstract: The Hendren's techniques of flap vaginoplasty for low confluence and vaginal pull-through for high confluence offer satisfactory cosmetic and functional results. Long-term follow-up is necessary to ensure social and psychosexual adjustment.
