Fibroplastic Parietal Endocarditis With Eosinophilia

Abstract: A 44-year-old woman with marked eosinophilia, leukocytosis, congestive heart failure, and the murmur of mitral stenosis had a restrictive type of pulse contour at cardiac catheterization. A right atrial angiogram revealed a huge right atrium, a small right ventricle, and a dilated contractile outflow tract consistent with the diagnosis of Loeffler's endocarditis. A marked conduction delay at the atrial level was demonstrated by His bundle electrogram studies.

“…The final step is to measure the distance using the "microlens parallax", π E ≡ π rel /θ E . This quantifies the amplitude of lens-source relative motion due to reflex motion of Earth's orbit (scaled to the Einstein radius) and therefore the amplitude of the lightcurve deviations due to this effect (see (11), Fig. 1 for a didactic explanation).…”

“…Endocarditis parietalis fibroplastica (EPF; Löffler's endomyocardial disease) and hypersensitivity myocarditis (HM) are entities accompanied by eosinophilic carditis. Whereas HM is a drug‐induced allergic reaction, EPF is mainly associated with prolonged eosinophilia 1,4 . The natural course of EPF shows severe damage to the heart which often leads to intractable heart failure and death.…”

“…The other form is called tropical endomyocardial fibrosis, which is mostly restricted to tropical areas in South America and Africa. Whereas the pathogenesis of the tropical form is unclear (parasitic, toxic), EPF represents a serious disease associated with blood eosinophilia due to different causes and is best understood as a complication of the latter 1 . In Western countries, hypereosinophilic syndrome (HES) is the most common underlying cause of eosinophilia but other circumstances presenting with eosinophilia, such as the Churg–Strauss syndrome, can also be triggers 2,3 .…”
