Fibula free flap in the treatment of mandibular osteoradionecrosis

Abstract: Micro-anastomosed FFF seems to be the method of choice for mandibular reconstruction in advanced mORN. Delayed treatment, however, seems to reduce the chances of good functional results. The precise indications for radical surgery and its timing remain to be defined, but it appears wise to decide on ever earlier radical surgery in the hope of obtaining better functional results.

“…Osteoradionecrosis (ORN) is a severe complication of radiotherapy in head and neck cancer patients. Up to 30% of the patients who undergo radiotherapy are affected by ORN, and at least, half of the patients who develop clinical signs and symptoms require surgical intervention (1,2). ORN has been described by Marx in 1983 as a chronic non-healing wound due to radiation causing hypoxic-hypocellular-hypovascular tissue, eventually leading to tissue breakdown.…”

“…The low scores of patients in mental health are largely related to the psychological reality of patients’ inability to accept jaw resection and their inability to adapt to changes in basic life functions such as speech and eating after surgery 19 . Patients are always worried about the recurrence of the tumor and the uncertainty of the future 20 , so they often experience symptoms such as irritability, fatigue, insomnia, pain, diarrhea and constipation 21 24 . This indicates that the emotional damage caused by the tumor to patients is more serious and long-lasting 25 .…”
