“…The upfront use of atherectomy to debulk thick calcium ring followed by lithoplasty to fracture the calcium has been described in case report and small series. 63) 64) 65) 66) After initial lesion preparation with either rotational or orbital atherectomy, delivery of lithoplasty balloon is enhanced and calcium fracture can be achieved at low inflation pressure with intravascular lithotripsy. Both Rota-Shock (rotational atherectomy followed by lithoplasty) and Orbital-Tripsy (orbital atherectomy followed by lithoplasty) appear to be feasible and safe with the limited current evidence, and more high-quality data from large-scale clinical trials is warranted.…”
