Flexible Intramedullary Nail Removal Using a Broken Screw Removal Set

Abstract: The use of flexible titanium intramedullary nails for management of pediatric long bone fractures and some adult fractures has become common. Nail removal after union can be challenging and often requires a larger exposure than nail placement to allow the insertion of grasping devices, such as pliers. A percutaneous technique for the removal of flexible intramedullary nails using extraction bolts from a broken screw removal set is presented. The technique is especially useful when the end of the nail is not pr… Show more

“…For this technique, availability of a cannulated reamer in the operating room is a prerequisite. Ferry and Dahners described the use of an extraction bolt from the Synthes screw removal set (Synthes 105.971, Paoli, PA, USA) [4]. The authors also described the use of hollow reamers from the screw removal set to overdrill the cut end of the nail.…”

“…A few strategies have been described in the literature for the removal of the elastic intramedullary nail. These techniques include the use of a bone chisel around of tip of the nail [ 5 ], the use of a hollow reamer and [ 6 ], and the use of a broken screw removal device [ 4 ]. Lascombes has described the use of the chisel followed by pliers [ 5 ].…”

“…372 Similarly, administration of licofelone to rats showed drastic decreases in the levels of inflammatory 5-Lox, Cox-1, and Cox-2 enzymes resulting in improved mechanical properties (load, stress, and stiffness; P < 0.05), compared to controls. 373 Conversely, several antibiotics and NSAIDs such as fluoroquinolone, 374 methylprednisolone, 375 betamethasone, 375 meloxicam, 376 celecoxib, 377 valdexicob, 377 and piroxicam, 377 acetaminophen, 377 have shown detrimental effects in varying degrees at normal dosages to healing tissue, resulting in suboptimal structural characteristics and biomechanics in the rotator cuff tendon. These studies thus suggest the need for careful selection and timing of drug administration for mitigating any detrimental effects of pharmacologic agents on the healing of rotator cuff tissue.…”
