Aim: Atherosclerosis and arteriosclerosis are mainly caused by the dysfunction of arterial components, namely, vascular endothelial cells, smooth muscle cells, and the extracellular matrix. Endothelial dysfunction is well established as a predictive surrogate marker of cardiovascular events; however, little is known regarding the clinical implications of vascular smooth muscle dysfunction for cardiovascular disease and microangiopathy. In the present study, we aimed to clarify the association of arterial dysfunction with micro-/macroangiopathy and conventional cardiovascular risk factors in 181 type 2 diabetic patients (T2DM; age±SD, 64±10 years; duration of diabetes, 12±10 years). Methods: Flow-mediated dilatation (FMD) and nitroglycerin-mediated dilatation (NMD) were assessed to evaluate endothelial dysfunction and vascular smooth muscle dysfunction, respectively, by using a novel ultrasound device, UNEXEF18G (Unex Co. Ltd., Japan). Results: The FMD and NMD were 6.4±3.9% and 13.4±6.6%, respectively. No significant differences in FMD were noted between T2DM with and without micro-or macroangiopathy; however, NMD in T2DM patients with micro-and macroangiopathy was significantly lower than that in T2DM patients without angiopathy. NMD decreased with the progression of chronic kidney disease (CKD) stage (p = 0.005), but not FMD (p = 0.071). On multiple regression analysis, significant independent contributors to FMD were age, smoking, systolic blood pressure, glycosylated hemoglobin, and serum total cholesterol, while those for NMD were age, systolic blood pressure, and waist circumference. Conclusion: The relationship of vascular complications and cardiovascular risk factors with NMD is different from that with FMD in type 2 diabetic patients.

SummaryWe investigated whether pre-germinated brown rice bran extract containing acylated steryl glucosides (PSG) reduces the risk of atherosclerosis in post-menopausal Vietnamese women. A total of 60 post-menopausal Vietnamese women (45-65 y old) with high LDL cholesterol levels (over 140 mg/dL) were randomly divided into PSG (n530) and placebo (n530) groups. The subjects in the PSG group were assigned a daily intake of 6 capsules containing 50 mg PSG, and the subjects in the placebo group were assigned a daily intake of 6 capsules containing corn oil for 6 mo. Before baseline and after month 2, month 4, and month 6 of the intervention, we conducted anthropometric measurements, blood biochemical examinations, a nutrition survey, and physical activity, flow-mediated dilation (FMD), and cardio-ankle vascular index (CAVI) measurements. Serum LDL cholesterol concentrations were significantly reduced from 163.6625.3 (mg/dL) to 135.9626.8 (mg/dL) compared to the placebo group (p,0.001). FMD values of the placebo group were significantly reduced from 6.665.1 (%) to 4.762.6 (%) compared to the PSG group (p,0.05). Tumor necrosis factor (TNF)-a concentrations in the PSG group were significantly reduced from 19.8611 (pg/mL) to 10.665.5 (pg/mL) compared to the placebo group (p,0.05). The findings suggest that PSG may improve LDL cholesterol, TNF-a levels, and FMD values. PSG might be considered in reducing the risk of atherosclerosis in post-menopausal Vietnamese women with high LDL cholesterol.

Abstract:"Vascular failure" is a new concept, which can be used interchangeably with "vascular dysfunction". It can be evaluated with several arterial properties, such as endothelial function, arterial stiffness, wave reflection, and intima-media thickness. The current Chinese hypertension guidelines recommend some of these vascular properties for risk assessment and stratification in the management of hypertension. However, none of these measurements are routine in the clinical setting in China, because of the lack of available devices and insufficient knowledge on the clinical efficacy of these vascular measurements. In order to improve the use of these vascular measurements in clinical practice, therapeutic research is required to prove reversibility of vascular failure by either pharmacological treatments or lifestyle modifications. Other research directions include identification of emerging risk factors, risk prediction and prevention with vascular measurements, and the development of newer, more accurate and efficient devices.
