Fracture union in percutaneous Kirschner wire fixation in paediatric tibial shaft fractures

Abstract: PurposeTo evaluate the efficacy of intramedullary Kirschner wires for the treatment of unstable tibial shaft fractures in children.MethodsThis prospective study was conducted at the Department of Orthopaedic Surgery in Maharishi Markandeshwar Medical College from June 2005 to June 2010. Sixty-six children having closed fracture of the tibial shaft with a mean age of 7.7 years (range, 2–14 years) were recruited from emergency and outpatient department. “…K-wire was rst introduced as an internal xation implant in 1909 by Martin Kirschner and it had been used for more than 110 years in orthopedic surgery [31]. Today K-wire is commonly used as a kind of temporary xation device in clinic, however it can also been used as definitive xator in displaced metacarpal shaft fractures, clavicle fractures and paediatric tibial shaft fractures [32][33][34]. The bone marrow cavity of bula is narrow and irregular in shape, so the K-wire can be used to x bula fracture according to the principle of multi-point xation [35].…”

“…Closed reduction with percutaneous Kirschner wire fixation and protection in a cast is useful for unstable, oblique fractures without comminution, 20 especially in younger children. This has been used in managing lower grade open fractures, as an alternative to external fixation.…”
