Free flap reconstruction of Achilles tendon and overlying skin defect using ALT and TFL fabricated chimeric flap

Abstract: A 33-year-old man developed a left Achilles tendon rupture and skin necrosis. We reconstructed the defect using an anterolateral thigh flap and a tensor fasciae lata muscle flap in a chimeric fashion. he was able to stand on a toe of the operated foot without help 6 months postoperatively.

“…Neurotization was described in four studies; connections included the posterior cutaneous nerve to the sural nerve branch 18 19 24 or tibial nerve 19 and the lateral femoral cutaneous nerve to the saphenous nerve. 25 Suture technique was only mentioned in three studies, with techniques including Kirchmayer's, 4 Krackow, 10 Figure of 8, 6 and Becker. 6 Operative time for composite flaps was mentioned in six studies, with an average time of 326.7 minutes (SD 23.1).…”

“…Excising a long tendinous muscle flap can also reconstuct the dynamic defect of the foot and ankle. For example, Ando et al 30 . applied ALT and TFL fabricated chimeric flaps to reconstruct Achilles tendon and overlaying skin defects successfully.…”

“…Excising a long tendinous muscle flap can also reconstuct the dynamic defect of the foot and ankle. For example, Ando et al 30 applied ALT and TFL fabricated chimeric flaps to reconstruct Achilles tendon and overlaying skin defects successfully. However, it is necessary to obtain the multi-leaf flap and muscle flap during the operation, which increases the time to separate the nutrient vessels and the risk of necrosis of the muscle flap.…”

“…29 The composite anterolateral thigh flap with the fascia lata has been widely applied in Achilles region, especially in massive deficit. [30][31][32] There are some unignorable drawbacks including vascular crisis, secondary thinning procedures for bulky flap and deficit of quadriceps femoris muscle contraction forces. 2,31 The radial forearm flap with palmaris longus tendon provides thin and pliable integument and long tendon, while it has significant donorsite morbidity and is better suited for small rather than massive defects.…”
