BackgroundClear cells are observed histopathologically in both benign and malignant neoplasms but their presence in salivary gland tumors has not been extensively documented.Material and MethodsWith IRB approval, the archive of the University of Florida College of Dentistry oral pathology biopsy service was retrospectively searched from 1994-2014 for all benign and malignant salivary tumors. Epidemiological data, tumor location and duration, and type of tumor were recorded. A four reviewer panel examined the original slides. Reviewers scaled each case as 0 (no clear cells present), 1 (few to focal clear cells), 2 (less than 50% clear cells), and 3 (greater than 50% clear cells).ResultsA total of 535 cases were included of which 48% of tumors displayed 0 clear cells (257/535), 31.4% (168/535) scored 1, 13.6% (73/535) scored 2, and 7% (37/535) scored 3. Of the 251 (47%) malignant neoplasms, 64% (160/251) demonstrated 0-1 clear cell change, while 36% (91/251) showed a score of 2-3. For the total 284 (53%) benign tumors, 93% (265/535) scored 0-1 and 7% (19/535) scored a 2-3 range. No statistical difference was noted for gender, age, or duration of time present in regards to presence or absence of clear cells. Statistically significant differences in clear cell presence were found between location groups, between benign and malignant diagnosis, and between specific diagnostic groups.ConclusionsThis study demonstrates the frequent presence of increased numbers of clear cells in oral salivary malignancies and highlights salivary gland differential diagnoses when presented with clear cell changes. Salivary gland tumors represent 3-11% of all head and neck cancers and 0.2% of all neoplasms. Among Poles, malignancies involving the large salivary glands constituted 0.3% of all malignancies in Poland and 9.7% of head and neck neoplasms. In 2014, the morbidity rate was higher among women, but over the past 15 years there has been a slight prevalence among men. As for benign neoplasms in Poland, the most commonly seen are pleomorphic adenoma and Warthin's tumor. Cystic glandular cancer, mucoid epidermal carcinoma and adenocarcinoma are among the most frequent malignancies. Less common are lymphoma and squamous cell carcinoma. Salivary gland neoplasms are usually located in the parotid gland, then in the sub-mandibular gland. The best prognosis is for women whose cancers are located in the parotid gland and those with lobular carcinoma. Figures have been on the rise lately, however. Deaths have been on a stable level.

Polymorphous low grade adenocarcinoma (PLGA) is the second most common malignant salivary gland tumour of the minor salivary glands. Human tissue kallikreins (KLKs) are a family of highly conserved serine proteases expressed by various tissues throughout the body. KLKs have become powerful tumour markers for the diagnosis of the cancer patient (e.g. PSA (KLK3)). The literature demonstrates a link between KLKs and salivary gland neoplasms. The purpose of this study is to determine levels of KLK mRNA in tissue samples of formalin fixed paraffin embedded polymorphous low grade adenocarcinoma (PLGA). Secondly, we wish to determine if KLK expression is limited to tumour cells alone.Nineteen cases of PLGA were reviewed . A diagnosis of PLGA was confirmed, demographic data was collected, and formalin fixed paraffin-embedded PLGA and normal salivary gland tissue samples were obtained. RNA isolation was achieved, followed by conversion to complementary DNA via reverse transcription. Synthesized DNA primers were added to target kallikrein DNA and through PCR, the quantitative level of expression of KLKs 1-15 was recorded. Samples exhibiting high and low KLK expression were selected for immunohistochemistry staining, using a standard protocol.Results from PCR data reveals statistically significant increase in the mean KLK mRNA expression for KLK1, KLK4, KLK10, KLK12 and KLK15 in PLGA tissue samples, as compared with normal salivary gland tissue (Mann Whitney U test, p<0.05).Immunohistochemistry results demonstrate tumour specific staining. Notably, all samples demonstrating relatively higher KLK mRNA expression showed equivalent or increased staining grade scores relative to the low KLK mRNA expression samples, with respect to a specific KLK.In tissue samples of polymorphous low grade adenocarcinoma there is an increase of KLK1, KLK4, KLK10, KLK12 and KLK15 mRNA relative to normal salivary gland tissue. Furthermore, the tumour cells stain positively and specifically for kallikreins.
