Frequent Embolization in Peripheral Angioplasty: Detection with an Embolism Protection Device (AngioGuard) and Electron Microscopy

Abstract: Microembolization of fibrin aggregates is a common incident in balloon angioplasty of femoropopliteal stenoses. Macroembolization occurred more frequently than previously reported. The use of embolism protection filters aided in the detection but not in the removal of larger emboli.

“…Most of the debris they collected appeared to be thrombotic and may have been related to a lack of anticoagulation prior to intervention. 11 Other investigators have evaluated the incidence of embolization associated with peripheral interventions. Kudo et al used Doppler techniques to analyze the incidence of ES during different facets of iliac artery intervention; comparing the different phases such as passage of the guidewire, balloon angioplasty, and stent deployment.…”

“…11 They collected debris in the filter but typically this was fibrin and red cells. The authors also typically observed macroemboli around and distal to the filter, which were not retrieved by the filter.…”

“…Alternatively, if microemboli from surgery account for the phenomenon, 18 then consideration could be given to the use and evaluation of embolic capture devices similar to those being considered in the context of coronary artery procedures and during carotid artery stenting. [21][22][23][24][25][26] Stork et al 18 reported that left sided CE was associated with an increased risk of microembolic events detected intraoperatively.…”

“…Distal embolization is a known complication of endovascular revascularization (1)(2)(3)(4). Distal embolization is also a well documented phenomenon in the setting of infrainguinal percutaneous balloon angioplasty, stent placement, thrombolysis, and endovascular atherectomy (3)(4)(5)(6).…”
