Photodynamic therapy (PDT) is widely used for the treatment of hypertrophic scars in clinical practice. However, the low transdermal delivery of photosensitizers in scar tissue and protective autophagy induced by Photodynamic therapy greatly reduces the therapeutic efficiency. Therefore, it is necessary to deal with these difficulties for overcoming obstacles in Photodynamic therapy treatment. In this study, a photosensitizer with photocatalytic performance was designed and synthesized using innovative MOFs (metal-organic frameworks). Additionally, the MOFs, together with an autophagy inhibitor chloroquine (CQ), was loaded in a high mechanical strength microneedle patch (MNP) for transdermal delivery. With these functionalized MNP, photosensitizers and chloroquine were delivered deep inside hypertrophic scars. Inhibition of autophagy increases the levels of reactive oxygen species (ROS) under high-intensity visible-light irradiation. Multiprong approaches have been used to remove obstacles in Photodynamic therapy and successfully enhance its anti-scarring effect. In vitro experiments indicated that the combined treatment increased the toxicity of hypertrophic scar fibroblasts (HSFs), downregulated the level of collagen type I expression as well as transforming growth factor-β1 (TGF-β1)expression, decreased the autophagy marker protein LC3II/I ratio, increased the expression of P62. In vivo experiments showed that the MNP had good puncture performance, and significant therapeutic effects were observed in the rabbit ear scar model. These results indicate that functionalized MNP has high potential clinical value.

Excessive extracellular matrix deposition drives fibroblasts into a state of high mechanical stress, exacerbating pathological fibrosis and hypertrophic scar formation, leading to tissue dysfunction. This study reports a minimally invasive and convenient approach to obtaining scarless tissue using a silk fibroin microneedle patch (SF MNs). We found that by tuning the MN size and density only, the biocompatible MNs significantly decreased the scar elevation index in the rabbit ear hypertrophic scar model and increased ultimate tensile strength close to regular skin. To advance our understanding of this recent approach, we built a fibroblast-populated collagen lattice system and finite element model to study MN-mediated cellular behavior of fibroblasts. We found that the MNs reduced the fibroblasts generated contraction and mechanical stress, as indicated by decreased expression of the mechanical sensitive gene ANKRD1. Specifically, SF MNs attenuated the integrin-FAK signaling and consequently down-regulated the expression of TGF-β1, α-SMA, collagen I, and fibronectin. It resulted in a low-stress microenvironment that helps to reduce scar formation significantly. Microneedles’ physical intervention via the mechanotherapeutic strategy is promising for scar-free wound healing.

Background Although micro-plasma radiofrequency (MPR) treatment has a significant effect on scars, patients require anesthesia to relieve the significant discomfort it produces. Whether anesthesia impacts efficacy is unclear. Objective To evaluate the effect of different anesthesia on MPR for hypertrophic scars. Methods A retrospective cohort study involving 101 people was conducted to investigate the effectiveness and safety of general and topical anesthetics for the treatment of MPR scars. The primary measures of outcome were the Vancouver Scar Scale (VSS) scores before the first treatment and six months after the last treatment, as well as the Visual Analogue Scale (VAS) scores on the day and the day after the final treatment. Results The differences in scar pigmentation, vascularity, and overall VSS scores were higher in the general anesthesia group than in the surface anesthesia group. Patients in the general anesthesia group had a lower pain level than those in the surface anesthesia group. After adjusting for confounding factors and propensity score matching, the outcome of VSS and VAS scores was stable. There was no statistical difference in the adverse effects and satisfaction between the two groups. Conclusion General anesthesia, as opposed to surface anesthesia, may not only ensure safety but also increase the effectiveness of MPR and lessen postoperative pain in the treatment of hypertrophic scars.
