Background: port-site metastases (PSM) have been reported following oncological laparoscopic surgery. However, their frequency after laparoscopic examination in gastric cancer has not been well established.Material and methods: prospective follow-up of 41 patients having had a staging laparoscopy and a follow-up longer than 12 months. Mean age was 65 years (29-89). After staging, an open gastrectomy was performed in 33 cases. Mean follow-up was 21.4 (12-66) months. PSM was defined as a node in the former portsite wound with adenocarcinoma histology at biopsy.Results: no patient showed clinical signs of PSM or port-site recurrence, even in advanced stages. We had no morbidity or postoperative mortality attributable to laparoscopic manoeuvres, and no need for laparotomy in cases without a gastrectomy indication.Conclusions: our results suggest that staging laparoscopy is a safe procedure in gastric carcinoma, as it is not associated with PSM after even considerable follow-up, and has a very low complication rate.

Г и н е к о л о г и я / G y n e c o l o g y Key words: ovarian cancer, laparoscopic approach, complications, rehabilitation period, relapse-free survivalСпоры по поводу возможности использования ла-пароскопического доступа в хирургическом лечении больных раком яичников (РЯ) продолжаются по сей день. Одно из первых сообщений относится к 1979 г. и принадлежит P. Spinelli и соавт., применившим ла-пароскопию в целях установления степени распро-странения и рестадирования у 95 пациенток с опухо-лями яичников [1]. Большинство онкологов критически относятся к применению лапароскопии в лечении РЯ, в первую очередь из-за невозможности соблюдения правил абластики, что приводит к быст-рому прогрессированию [2][3][4]. Большое количество рецидивов, port-site-метастазов, развивающихся даже при начальном РЯ, заставляет с настороженностью относиться к подобным операциям [5].Нами проанализирован опыт лечения 134 больных с пограничными опухолями и начальным РЯ, из них 48 пациенткам операции были выполнены лапароско-пическим доступом. Оговоримся, что под термином «начальный рак» рассматривались случаи I стадии РЯ, при которых возможно выполнение органосохраняю-щих операций. Принятое в современных онкологиче-ских статистических сборниках объединение в одну группу «начального» РЯ больных с I и II стадиями ав-тору представляется нецелесообразным, поскольку и подходы к выбору лечебной тактики, и отдаленные результаты лечения в данных клинических ситуациях значительно разнятся.Все 134 пациентки подверглись хирургическому вмешательству в различных объемах. Рандомизации больных по выбору лапаротомного либо лапароскопи-ческого доступа не проводили, что, безусловно, может послужить поводом для критики.
