Background Laparoscopic sleeve gastrectomy (SG) has surpassed Roux-en-Y gastric bypass (RYGB) as the most prevalent bariatric procedure worldwide. While RYGB and SG demonstrate equivalent short-term weight loss, long-term weight loss tends to be greater after RYGB. Differences in the effect of these procedures on gastrointestinal hormones that regulate energy homeostasis are felt to partially underlie differences in outcomes. The objective of this study was to prospectively quantify blood levels of gut hormones of energy and glucose homeostasis at one year follow up to delineate possible reasons for greater efficacy of RYGB over SG in achieving weight loss. Methods Patients undergoing SG (n = 19) and RYGB (n = 40) were studied before surgery and at 2,12, 26, and 52 weeks postoperatively. Blood samples drawn in the fasted state and after a liquid mixed meal were assayed at baseline, 26, and 52 weeks for peptide YY (PYY), glucagon-like peptide-1 (GLP-1), ghrelin, insulin, glucose, and leptin. Fasting and postprandial appetitive sensations were assessed by visual analog scale. Results At 1 year there was greater weight loss in RYGB compared with SG patients (30% vs 27%; P = 0.03). Area under the curve (AUC) after the mixed meal for PYY was greater in RYGB patients (P<0.001). RYGB patients had significant increases in GLP-1 AUC compared to baseline (P = 0.002). Ghrelin levels decreased only after SG compared to baseline (P<0.001) but were not significantly different from RYGB. There was a trend toward decreased sweet cravings in RYGB patients (P = 0.056).

Stress, mood, and eating behavior play an important role in appetite and weight regulation. In particular, ghrelin, as the only known orexigenic hormone, has been suggested to be an influential mediator in food intake responses to stress. The exact role of ghrelin in the hypothalamic–pituitary–adrenal axis is still unknown and further challenged by the psychological aspects of stress and eating behavior. This study aimed to assess the effect of chronic stress and subjective concern about eating on acute stress-induced changes in acylated ghrelin. In a 2-day study, sixteen healthy male participants were confronted with a stressful situation as well as a control situation. Additional measurements of heart rate, subjective hunger ratings, and subjective mood ratings were made to assess successful acute stress induction. The linear mixed model approach revealed a significant effect of acute stress on acylated ghrelin for a study-day*chronic-stress interaction (p < 0.001). Concern about eating did not affect acylated ghrelin levels after acute stress exposure. The significant interaction showed that lower chronic stress exposure was associated with a stronger acylated ghrelin response after acute stress exposure versus control condition. At the same time, participants with higher chronic stress exposure showed a blunted acylated ghrelin response after acute stress exposure compared to the control situation. Our findings indicate that chronic stress exposure can influence acylated ghrelin response after acute stress encounters, possibly affecting subsequent food intake and explaining the often diverse outcome in measurements of acute stress responses.

Objective: When abnormalities in kidney structure or function are evident for more than three months, it is referred to as chronic kidney disease (CKD), a complicated and progressive chronic disorder that can have devastating health effects. Either reduced glomerular filtration rate or indicators of renal injury could be evident. End-stage renal disease (ESRD) patients are increasingly choosing PD, HD, and kidney transplantation as treatments to extend their lives because of the development of renal replacement therapies, such as kidney transplantation, hemodialysis (HD), peritoneal dialysis (PD), and continuous renal replacement therapy (which is frequently used for acute renal failure). Loss of muscle mass may result from dialysis treatments that increase protein breakdown and decrease protein synthesis. These reactions continue after dialysis. The purpose of this research is to look into the connection between the ghrelin. Materials and Method: In order to determine the blood lipid profile and Ghrelin hormone levels, 90 individuals with Chronic Kidney Disease (CKD) on dialysis were divided into 45 males, 45 females, and 90 healthy subjects. The Ghrelin hormone concentration was compared with the other biochemical parameters, and the relationship between the hormones Ghrelin and Nitric Oxide was also found. Results: Ninety healthy subjects and ninety chronic kidney disease (CKD) patients with dialysis had their blood lipid profiles and Ghrelin hormone and nitric oxide levels measured. The results included a comparison of the hormone's concentration with other biochemical parameters and the identification of the hormone's relationship with nitric oxide. Results: When compared to the control group, dialysis patients' nitric oxide levels significantly decreased (p≤0.05). There were no discernible variations in the levels of serum nitric oxide between the sexes. (p < 0.05).Ghrelin levels in the blood serum of dialysis patients were significantly lower than those in the control group (p≤0.05). Additionally, it was demonstrated that the level of the hormone Ghrelin was significantly lower in females than in males (p≤0.05). Conclusion: When comparing dialysis patients to the control group, we detect a significant drop in the levels of Ghrelin Hormone and Nitric Oxide in patients with chronic kidney disease.It follows that low nitric oxide and ghrelin levels in chronic renal disease patients can cause kidney failure to progress to an advanced degree and eventually require dialysis.
