Gênero, vulnerabilidade das mulheres ao HIV/Aids e ações de prevenção em bairro da periferia de Teresina, Piauí, Brasil

Abstract

ResumoCom a mudança ocorrida no curso da epidemia de Aids a partir dos anos 1990, marcada pelo aumento da incidência entre segmentos fora dos "grupos de risco", entre eles as mulheres, o conceito de risco individual foi substituído pela noção de vulnerabilidade social, abrangendo questões relacionadas ao comportamento coletivo, como as relações de gênero, além das ações do Estado voltadas para essas questões. Este artigo relata os achados de uma pesquisa de corte qualitativo realizada com mulheres adultas, vivendo relações conjugais estáveis, moradoras de bairro considerado de baixa renda, na periferia de Teresina. Para isso, buscou-se identificar a vulnerabilidade desse grupo à infecção pelo HIV, em função da utilização ou não de preservativo, bem como a relação desse aspecto com as relações de gênero vivenciadas pelos parceiros e o papel do Programa Saúde da Família nesse contexto. Verificou-se que a maioria das mulheres não faz uso contínuo do preservativo, por estar vivendo relação estável com parceiro fixo; entretanto, considera-se vulnerável, pois não tem absoluta confiança no comportamento sexual do parceiro. A negociação com o parceiro sobre o uso do preservativo é quase sempre difícil, e, por vezes, requer a alegação de que representa segurança para se evitar uma gravidez indesejada. Rassalta-se que o Programa Saúde da Família não incorpora a discussão sobre sexualidade e relações de gênero ao trabalho de educação em saúde da mulher por estar centrado no acompanhamento pré-natal e na redução do câncer ginecológico.
