Context: Scopolamine, a muscarinic receptor antagonist, causes memory loss that resembles Alzheimer's disease (AD). Echium amoenum L. (Boraginaceae) is a famous medicinal plant of Iran that is traditionally used as a sedative and mood enhancer.Objective: This study evaluates the effect of hydroalcoholic extract of E. amoenum flowers on scopolamine-induced memory impairment in rats.Materials and methods: Fifty male Wistar rats were randomly divided into five groups. Control group received normal saline, model group received scopolamine (0.7 mg/kg, IP, daily for 21 days), and test groups received E. amoenum extract (50, 75, and 100 mg/kg, IP, daily for 21 days) 30 min before each scopolamine injection. The elevated plus maze (EPM), shuttle box, novel object and rotarod tests were performed after treatment. Brain levels of malondialdehyde (MDA) and total antioxidant capacity (TCA) were also determined.Results: Scopolamine-treated rats spent more time exploring the novel object compared to the control, and E. amoenum extract at all three doses significantly decreased the time spent exploring the novel object (p < 0.05). E. amoenum extract (75 and 100 mg/kg) significantly elongated the secondary latency in rats receiving scopolamine in the shuttle box test (p < 0.05). In addition, treatment with 75 and 100 mg/kg doses of E. amoenum extract significantly ameliorated scopolamine-induced motor in coordination in rotarod test (p < 0.05). It also significantly increased the time spent in the open arms and reduced the time spent in the closed arms of EPM (p < 0.05). Treatment of scopolamine-exposed rats with E. amoenum extract significantly increased TCA and reduced MDA level of brain (p < 0.05).Discussion and conclusions:E. amoenum extract shows protective effect against scopolamine-induced impairment and is suggested to be tested in clinical trials to evaluate the efficacy on AD.

BackgroundAs a drug originally introduced for its anticonvulsant effects, gabapentin has been recently shown to be effective in the treatment of nausea and vomiting in various clinical settings. This study compared the antiemetic efficacy of oral gabapentin, intravenous ramosetron and gabapentin plus ramosetron in patients receiving fentanyl-based patient-controlled analgesia after laparoscopic gynecologic surgery.MethodsOne hundred and thirty two patients undergoing laparoscopic gynecologic surgery under general anesthesia were allocated randomly into three groups: group G received 300 mg oral gabapentin 1 h before anesthesia, group R received 0.3 mg intravenous ramosetron at the end of surgery, and group GR received a combination of 300 mg oral gabapentin 1 h before anesthesia and 0.3 mg intravenous ramosetron at the end of surgery. Postoperative nausea, retching, vomiting, rescue antiemetic drug use, pain, rescue analgesic requirements and adverse effects were assessed at 0–2, 2–24 and 24–48 h after surgery. Postoperative nausea and vomiting (PONV) was defined as the presence of nausea, retching or vomiting.ResultsThe incidence of complete response (no PONV and no rescue antiemetics up to 48 h postoperatively) was significantly higher in group GR (26/40, 65%) than group G (16/40, 40%; P = 0.025) and group R (18/44, 41%; P = 0.027), whereas there was no significant difference between group G and group R (P = 0.932). There were no significant between-group differences in the incidence of emetic episodes, use of rescue antiemetics, severe emesis, use of rescue analgesics or any adverse effects. Postoperative pain scores were also similar among groups.ConclusionsThe combination with gabapentin and ramosetron is superior to either drug alone for prevention of PONV after laparoscopic gynecologic surgery.Trial registrationClinicalTrials.gov NCT02617121, registered November 25, 2015.

Background:Development of new multimodal analgesic regimens have led to substantial improvement in postoperative pain relief. We designed this study to compare the effect of combined vitamin B complex—gabapentin versus gabapentin alone on postoperative pain in women undergoing cesarean section under spinal anesthesia.Methods:One hundred twenty-eight women who underwent cesarean section under spinal anesthesia were randomized to receive orally 300 mg gabapentin (group G) or 300 mg of gabapentin plus 2 vitamin B complex (group GB) tablets 30 minutes before surgery. Postoperative pain intensity and total analgesic consumption during 12 hours after surgery, vomiting, and drowsiness during recovery were assessed.Results:The pain intensity in the gabapentin plus vitamin B complex group was lower than gabapentin group during 12 hours after surgery (95% CI: 1.4–2.2; P < .001). Meanwhile, the total analgesic consumption in this group was less than gabapentin alone (95% CI: 1.07–1.24; P = 0.034). The incidence of vomiting in patients who receive combined gabapentin—vitamin B complex group was similar to gabapentin alone (P = .206). The difference of the distribution of the relative frequency of sedation according to Ramsay sedation scores in patients between 2 groups were insignificant (P = .82). All newborns in our study were free of any adverse effects.Conclusion:Addition of vitamin B complex to gabapentin reduced intensity of postoperative pain and also the total amount of analgesic consumption within the first 12 hours postoperative following cesarean section.
