Uterine leiomyomas are a common finding in medical practice, but their frequency changes drastically when contextualized in a syndrome, as in the following case. A 50-year-old woman with a known Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome presented at our clinic with abdominal pain located in the lower quadrants and scarcely responsive to analgesic therapy. A twisted gynecological pelvic mass was diagnosed, and management for prompt resolution was adopted. Histologically the mass was described as a leiomyoma. The aim of the present study is to share our experience and to review the literature to compare different manifestation of the disease and different approach used in the various centers. The additional novelty of the paper is the immunohistochemical study we carried out on the leiomyoma that is contrasted with the current etiopathogenetic theories.

Tóm tắt: Hội chứng MRKH là một hình thái dị dạng sinh dục hiếm gặp (1/4000-5000 người) biểu hiện không có tử cung, dù 2 phần phụ vẫn phát triển bình thường. Những người mắc hội chứng MRKH không có hiện tượng kinh nguyệt mặc dù các dấu hiệu sinh dục phụ hoàn toàn bình thường. Tình trạng các tế bào xơ ở dải xơ di tích của ống Muller phát triển thành các khối u giống khối u xơ tử cung rất hiếm gặp và thường gây khó khăn trong chẩn đoán nguồn gốc u. Phẫu thuật cắt bỏ khối u để tránh hiện tượng chèn ép do u gây nên là giải pháp điều trị hiệu quả nhất.Từ khóa: Hội chứng MRKH, u xơ tử cung. Giới thiệu chung U xơ xuất phát từ các dải xơ hoặc từ di tích của sừng tử cung không phát triển trong hội chứng Mayer Rokitanski Kuster Hauser (MRKH) là một hình thái bệnh lý vô cùng hiếm gặp. Các khối nhân xơ thường phát triển trong tiểu khung cho đến một kích thước đủ lớn gây chèn ép trong các tạng tiểu khung và tạo nên triệu chứng như đau hoặc rối loạn đại tiểu tiện, khiến bệnh nhân đi khám và được phát hiện bệnh. Sau đây là một trường hợp hi hữu như vậy được phát hiện tại Bệnh viện Phụ Sản Hà Nội năm 2017. Ca lâm sàngBệnh nhân nữ 29 tuổi đã được chẩn đoán mắc hội chứng Mayer Rokitanski Kuster Hauser (MRKH), không có tử cung, có 2 buồng trứng và có âm đạongắn hơn bình thường [1]. Bệnh nhânlà người vô kinh nguyên phát và không mắc bệnh lý phụ khoa từ trước. Tháng 8/2017, bệnh nhân đến khám tại bệnh viện Phụ Sản Hà Nội vì nặng tức bụng và siêu âm phát hiện một khối u to ở tiểu khung, bệnh nhân được nhập viện. Kết quả khám bệnh cho thấy bệnh nhân có kiểu hình nữ, các dấu hiệu sinh dục phụ mang tính nữ hoàn toàn bình thường. Âm hộ bình thường, âm đạo bịt không có cổ tử cung dài khoảng 5 cm. Thăm âm đạo xác định thấy khối u chắc, ranh giới rõ kích thước 10x12 cm trong tiểu khung. Tại thời điểm khám, chúng tôi tiến hành siêu âm đường âm đạo và
