Guidelines for the Appropriate Use of Bedside General and Cardiac Ultrasonography in the Evaluation of Critically Ill Patients—Part II: Cardiac Ultrasonography

Abstract: There was strong agreement among a large cohort of international experts regarding several class 1 recommendations for the use of bedside cardiac ultrasound, echocardiography, in the ICU. Evidence-based recommendations regarding the appropriate use of this technology are a step toward improving patient outcomes in relevant patients and guiding appropriate integration of ultrasound into critical care practice.

“…However, this cohort of severe sepsis and septic shock are less likely to be missed with either definition because they comprise the extreme spectrum of illness 39. The study duration also correlated with the evolution of critical care ultrasonography and changes in healthcare delivery at the Mayo Clinic, both of which conceivably could have influenced our study results 40, 41. Finally, the single‐region, single‐institution, and referral center nature of the Mayo Clinic could impact the generalizability of findings to other populations.…”

“…This can be explained by multiple hypotheses. First, this study duration coincided with the evolution of point‐of‐care cardiac ultrasonography in critical illness, resulting in the higher application and likely greater diagnosis of TTC at the bedside 29. Second, the initial TTC criteria were proposed in 2004 with a subsequent revision in 2008 that potentially resulted in the further unification of clinical definitions and higher awareness in treating physicians 2, 5.…”

“…Several major societies have issued guidelines and established courses and certifications [3,[21][22][23]. The need for standardized training, a defined curriculum, and clear definitions of competency has clearly been recognized, but little consensus exists on the exact hours of training required.…”
