Hallucinogen Persisting Perception Disorder and Risk of Suicide

Abstract: A 30-year-old male patient developed a hallucinogen persisting perception disorder (HPPD) after smoking cannabis laced with phencyclidine (PCP) or lysergic acid diethylamide (LSD) 10 years prior to hospital admission. Clinically, he reported seeing vivid, saturated colors and caricature-like objects. The patient described perceiving objects or people in motion as moving faster than normal. He reported living in a dream-like state and feeling numb and detached from other people and his surroundings. Upon pharma… Show more

“…The use of cannabis has been associated with the development of perceptual distortions in seven patients [ 29 , 46 , 48 , 49 , 61 ]. In one case, it was associated with 3,4-Methylenedioxymethamphetamine (MDMA) and in another case with PCP [ 48 , 49 ]. In two patients, visual distortion followed the consumption of synthetic cannabinoids [ 61 ].…”

“…A case report described an HPPD patient with a history of bipolar disorder, a previous consumption of LSD, cannabis, cocaine, and phencyclidine (PCP) ( 30 ). The patient, even though treated with lamotrigine, citalopram, and mirtazapine, committed suicide, after the hospitalization ( 31 ).…”

“…Furthermore, the pharmacotherapy of HPPD may be different from study to study, as only few studies have been published so far and any recommendations are based almost entirely on non-controlled studies of small patient populations or single case reports ( 3 , 18 31 ). Risperidone is a highly potent antagonist of both postsynaptic 5-HT 2 and dopamine D 2 receptors ( 62 ).…”

“…13 A hallucinogen persisting perception disorder (HPPD) was reported after 10 years of taking LSD with severe psychopathology, war exposer, and previous suicidal behavior. 14 However, a recent systematic review assessing the relationship between psychedelics and suicidality found positive, negative, and mixed findings. 15 Therefore, further evidence is needed to find out the cause and effect association (if any).…”
