Has the EAGLE landed for the use of clear lens extraction in angle-closure glaucoma? And how should primary angle-closure suspects be treated?

Abstract: Angle-closure glaucoma is an aggressive condition that causes millions to become blind worldwide. This review explores the use of prophylactic laser peripheral iridotomy (PI) in patients classified as primary angle-closure suspects (PACS), and additionally, the use of clear lens exchange as a primary treatment option in established angle-closure disease with or without glaucoma. As PI has a strong prophylactic effect in fellow eyes of patients who have had an acute attack, its use has been widely adopted in th… Show more

“…Acute glaucoma, which remains the most common glaucoma type among Asians, is predicted to affect 20 million people worldwide and is characterized by a sudden and substantial increase in intraocular pressure (IOP), severe eye pain, and irreversible vision loss that can progress to permanent blindness [1][2][3]. The rapid elevation in IOP leads to retinal ischemia/ reperfusion (RIR) injury and retinal ganglion cells (RGCs) death [4].…”

“…Initial treatment options include Laser Peripheral Iridotomy (LPI), surgical peripheral iridectomy, and Argon Laser Peripheral Iridoplasty (ALPI). [13] Iridotomy is necessary to relieve the pupillary block and reduce to lower the IOP. [14] Refractive surgery such as precise lens extraction may benefit high myopic eyes because the thick lens predisposes patients to angle-closure glaucoma.…”
