Hb Hope: A Rare Variant of Haemoglobin-Forged a Rise in HbA1c on Immunoturbidimetric Assay

Abstract: Glycosylated hemoglobin (HbA1c) determination is a powerful means for assessing the evaluation and management of patients with diabetes mellitus. Hemoglobin (Hb) variants and chemically modified derivatives of Hb can affect the accuracy of measurement of HbA1c done by various analytical methods. We report a patient with a rare variant of Hb (Hb Hope) that caused an abnormally high value of HBA1c when assayed using immunoturbidimetric assay ("Tina-quant" 2nd generation assay) and also elucidate the nature of th… Show more

“…Therefore, the possibility of interference by a silent Hb like Hb hope should always be discussed while analyzing the abnormally raised HbA1c value. Moreover, several methods for estimation of glycosylated Hb should be used while dealing with aberrant raised HbA1c value 6 . The laboratories should have alternative assay systems such as glycated albumin to assist the management of the DM patient with a hemoglobin variant.…”

“…The same mutation at the analogous position in the beta subunit of HbA (N108K) is known to decrease oxygen affinity . In addition, LtEc also has three other substitutions relative to EfEc (H103Q in the A subunit, along with Q132F and G145D in the D subunit) that have been shown to decrease the oxygen affinity of HbA (D94H, P124Q, and G136D, respectively) . Overall, any or all of these isolated mutations could be responsible for the relatively low oxygen affinity of LtEc, but future mutational studies would be needed to confirm this hypothesis.…”

“…1 Hb I Philadelphia, Hb Hope and Hb Sherwood Forest were associated with falsely elevated HbA 1C levels. 10,26,28,29 The first case of heterozygous Hb Sherwood Forest was reported in a pregnant female having anemia and another case of homozygous Hb Sherwood Forest had erythrocytosis. Majority of the cases were reported from Pakistan and Iran.…”
