Heart Failure Associated with Metastatic Myocardial Calcification in a Hemodialysis Patient with Progressive Calcification of the Hand

Abstract: Metastatic myocardial calcification is a frequent cause of heart failure in hemodialysis patients. However, early detection is difficult, often resulting in a poor prognosis. A 47-year-old man with hemodialysis-dependent end-stage renal disease presented with progressive dyspnea. Levels of serum phosphate, calcium, and intact parathyroid hormone were poorly controlled. He developed pain in his right thumb 1 year before presentation, and the pain gradually increased and extended to the entire right hand. Hand r… Show more

“…Metastatic calcifications are a frequent cause of heart failure in patients on chronic hemodialysis 14 and have been reported to cause sudden cardiac death. 15 Calcifications may also cause focal wall motion abnormalities, 11 and lifethreatening arrhythmias can occur if there is calcification of the conduction system.…”

“…It is possible that the diastolic dysfunction caused by hypertrophy further worsened due to the development of myocardial calcification. In other reports, myocardial calcification was identified at the time of onset of symptoms of acute heart failure, and the patient's prognosis was poor (2). It is difficult to detect myocardial calcification on conventional chest radiography, and clinical signs suggestive of myocardial calcification remain unclear.…”

“…In each case, severe myocardial calcification was detected following the onset of symptoms of acute heart failure and the patient's prognosis was poor, as early detection is difficult (2). The precise cause of myocardial calcification and its effects on cardiac hemodynamics remain unclear.…”

“…Bildirilen metastatik myokardial kalsifikasyonlu vakalarda, kalp yetmezliği geliştiğinde kötü prognozla seyretmektedir. Yaşayan hastalarda konvansiyonel bilgisayar tomografi tanı koymada yardımcı olmaktadır (4). Hemodiyaliz hastalarında kalp yetmezliği geliştiğinde nadir de olsa metastatik myokardial kalsifikasyon akla getirilmelidir.…”
