Heart involvement in lymphomas. The value of magnetic resonance imaging and two-dimensional echocardiography at disease presentation

Abstract: Background and Methods. Thirty‐six patients with mediastinal lymphoma were studied with chest magnetic resonance imaging (MRI) and two‐dimensional echocardiography at presentation to define the extent of the disease in the paracardiac area. Results. Involvement of cardiac structures was present in 23 of 36 patients (64%). Pericardial contiguity was detected in 23 of 23 patients (100%) by MRI and in 18 of 23 patients (78%) by echocardiography. Pericardial effusion, present in 17 patients (74%), and pericardial … Show more

“…The high sensitivity and specificity of echocardiography and magnetic resonance imaging, as well as multidetector CT, are helpful in making the diagnosis. [8,9] It is not always possible to differentiate between benign and malignant lesions based on a histopathological examination of specimens obtained through a fine-needle aspiration biopsy. The tissue sample must be of adequate size, and in addition to a classic histological examination, cytogenetic, immune analysis, and immunophenotypic examination should be also performed.…”

“…This has been felt to be partly due to the early detection of the tumour in these cases together with the use of aggressive multi‐modal therapy. The value of serial two‐dimensional echocardiography as well as MRI in the detection and diagnosis of a cardiac mass of uncertain origin should not be underestimated (12). The use of appropriate diagnostic procedures (such or pericardial aspirates or trans/intra‐thoracic biopsies) to obtain a tissue diagnosis is equally important.…”

“…If necessary, these images can be acquired over several separate breath-hold acquisitions. Tesoro-Tess and colleagues studied 36 patients with lymphoma and noted that cardiac involvement was common, occurring in 64% of patients, and was more frequently noted on CMR than on echocardiography, emphasizing the importance of high resolution anatomic imaging (184).…”
