Измеренные интервалы при помощи различных математических формул (средне-квадратичного отклонения, преобразования Фурье и др.) преобразуются в определенные показатели, которые изменяются при различных отклонениях вегетативного профиля как в условиях нормы, так и при патологии.Методика оценки вариабельности ритма сердца широко применятся для диагностики, прогнозирования и лучшего понимания взаимодействия звеньев вегета-тивной системы при патологии нервной [1] и сердечно-сосудистой систем [2]. Что же касается сердечно-сосудистой системы, то изменениям ВСР при различной патологии посвящено множество работ, носящих подчас противоречивый характер.Ни при одном патологическом состоянии сердца нет такого единодушия во мнении, как при сердечной недо-статочности (СН), при которой происходит увеличение симпатической активности, учащение сердечного ритма и повышение уровня циркулирующих катехоламинов. По мнению подавляющего большинства авторов, у пациен-тов с СН устойчиво наблюдается снижение ВСР [5,6,7,9]. Но сообщения о взаимоотношениях между измене-ниями ВСР и степенью левожелудочковой дисфункции носят противоречивый характер [5,8]. В самом деле, в то время как уменьшение временных характеристик ВСР соответствует тяжести заболевания, взаимоотношения между спектральными компонентами и показателями желудочковой дисфункции более сложны. Так, например, у большинства больных в далеко зашедшей фазе забо-левания и резко сниженной ВСР НЧ-компонент вообще не выявляется, несмотря на клинические признаки сим-патической активации. Таким образом, представляется, что в состояниях, характеризующихся устойчивой и не встречающей сопротивления активацией симпатическо-го звена, чувствительность синусового узла к нервным влияниям существенно снижается [8]. С другой стороны, отмечена высокая корреляция активности парасимпати-ческой системы и фракции выброса, которая определяет уровень снижения насосной функции сердца [10].При анализе частотных параметров ВСР в ходе ис-следования, включившего 25 человек с хронической недостаточностью кровообращения III-IV по NYHA и 21 здорового, было продемонстрировано, что спектральная мощность всех частот у больных с сердечной недоста-точностью уменьшается, особенно ОНЧ [11]. При недо-статочности кровообращения (НК) имеется пониженная вагусная, но относительно сохраненная симпатическая модуляция частоты сердечных сокращений (ЧСС). Реферат. Определена динамика показателей вариабельности ритма сердца в ответ на физиологическую нагруз-ку в группе больных с сердечной недостаточностью в сравнении с контрольной группой. SummaryBuckgrouncl: In contrast to postinfarct patients, little is known about cardiac autonomic tone and its relation to spontaneous ventricular tachyarrhythmias in idiopathic dilated cardiomyopathy (IDC). Both heart rate variability (HRV) and baroreflex sensitivity (BRS) are indices of autonomic innervation of the heart.Hypothesis: The aim of the present study was to determine the relation between cardiac autonomic tone assessed by HRV and BRS and spontaneous nonsustained ventricular tachycardia (NSVT) on Holter in a large patient population with IDC.Methods: 24-h digital Holter recordings including HRV analysis and BRS testing were prospectively performed in I37 patients with IDC and preserved sinus rhythm. Mean age was 48 k 12 years, and mean left ventricular (LV) ejection fraction was 32 f 9%. The HRV analysis on Holter included the mean RR interval (RRm), the standard deviation of all normal RR intervals (SDNN), the square root of the mean of the squared differences between adjacent normal RR intervals (rMSSD), and the proportion of adjacent normal RR intervals differing more than 50 ms (pNN50). Testing for BRS was performed noninvasively using the phenylephrine method.Results: Of 137 study patients, 42 (3 1 %) had spontaneous NSVT on 24-h Holter. Conclusions: Patients with IDC and spontaneous NSVT on Holter are characterized by a higher NYHA functional class, a lower LV ejection fraction, an increased LV end-diastolic diameter, and a tendency toward a lower SDNN value compared with patients without NSVT. The remaining measures of HRV including rMSSD and pNN50 reflecting primarily tonic vagal activity, as well as BRS reflecting predominantly reflex vagal activity, were similar in patients with and without NSVT. The prognostic significance of these findings in patients with IDC is currently under investigation in the Marburg Cardiomyopathy Study (MACAS) at our institution.
