Heart rate variability modification as a predictive factor of sudden unexpected death in epilepsy: How far are we? A systematic review and meta‐analysis

Abstract: Background: Sudden unexpected death in epilepsy (SUDEP) is a sudden, unexpected death in people with epilepsy, with or without evidence of an epileptic seizure. The pathophysiological mechanism underlying SUDEP appears to be partly associated with an autonomic nervous system (ANS) dysfunction. Heart rate variability (HRV) analysis is a reliable, non-invasive method for detecting fluctuations in the ANS. In this systematic review we analyzed the data available in the literature on changes in HRV parameters in p… Show more

“…HRV is the fluctuation of the time interval between adjacent R waves on ECG [ 12 ]. It can reflect the change of difference in successive heartbeat cycles, a reliable and non-invasive method for detecting fluctuations in the autonomic nervous system (ANS) [ 13 ]. Low HRV is associated with a poor prognosis in many diseases [ 5 , 14 , 15 ] and can also reflect the state of the brain [ 16 ].…”

“…The Polar flow sync app was used to export the raw data, and the HRV analysis was done using the Kubios HRV Standard software (3.3.1). To provide insights into the autonomic nervous system's influence on cardiac regulation during JPS tasks, a widely used 36 , 37 index, the ratio of low frequency to high frequency (LF/HF) of HRV was used. The normal value of LF/HF is 2.8 ± 2.6.…”

“…The dysregulation of multiple neurotransmitters, including but not limited to GABA and glutamate, can potentially disrupt the delicate equilibrium between excitatory and inhibitory processes in respiratory centers. The modulation of respiratory regulation may be influenced by alterations in neurotransmitters such as serotonin and acetylcholine, among others [ 22 ]. To summarize, the presence of respiratory failure in epileptic encephalopathies is a complex phenomenon that involves various aspects of neurobiology, central nervous system function, autonomic regulation, and molecular mechanisms.…”
