Background: The global burden of trauma disproportionately affects low-income countries and middle-income countries (LMIC), with variability in trauma systems between countries. Military and civilian healthcare systems have a shared interest in building trauma capacity for use during peace and war. However, in LMICs it is largely unknown if and how these entities work together. Understanding the successful integration of these systems can inform partnerships that can strengthen trauma care. This scoping review aims to identify examples of military-civilian trauma systems integration and describe the methods, domains, and indicators associated with integration including barriers and facilitators. Methods: A scoping review of all appropriate databases was performed to identify papers with evidence of military and civilian trauma systems integration. After manuscripts were selected for inclusion, relevant data was extracted and coded into methods of integration, domains of integration, and collected information regarding indicators of integration, which were further categorized into facilitators or barriers. Results: Seventy-four studies were included with authors from 18 countries describing experiences in 23 countries. There was a predominance of authorship and experiences from High-Income Countries (91.9 and 75.7%, respectively). Five key domains of integration were identified; Academic Integration was the most common (45.9%). Among indicators, the most common facilitator was administrative support and the lack of this was the most common barrier. The most common method of integration was Collaboration (50%). Conclusion: Current evidence demonstrates the existence of military and civilian trauma systems integration in several countries. High-income country data dominates the literature, and thus a more robust understanding of trauma systems integration, inclusive of all geographic locations and income statuses, is necessary prior to development of a framework to guide integration. Nonetheless, the facilitators identified in this study describe the factors and environment in which integration is feasible and highlight optimal indicators of entry.

Objetivo: mapear las competencias del enfermero aeroespacial en situaciones de desastre. Método: revisión de alcance que siguió los pasos recomendados por el JBI y la checklist Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR). Fue realizada en tres fases por dos revisores independientes, con cegamiento y con el apoyo de un tercer revisor para resolver desacuerdos. La selección se basó en el análisis de títulos, descriptores y resúmenes, con criterios de elegibilidad específicos, seguida de la lectura completa de los textos. Al final de la fase de selección se incluyeron 37 publicaciones. Resultados: los resultados indicaron que es necesario desarrollar habilidades técnicas, tener conocimiento sobre fisiología de vuelo, estar familiarizado con los procedimientos de emergencia aeronáuticos, tener habilidades de comunicación, liderazgo y responsabilidades en la preparación de aeronaves. Durante el transporte, los enfermeros realizan anamnesis, exámenes físicos, monitoreo de pacientes, procedimientos clínicos y gestionan las complicaciones. Después del vuelo, realizan registros, procedimientos, higienizan el equipamiento clínico y reponen los insumos. Conclusión: debido a la complejidad de las prácticas de enfermería aeroespacial en situaciones de desastre, es necesario que los profesionales desarrollen competencias para garantizar una atención segura y eficaz. Hay que desarrollar tecnologías, marcos regulatorios y disposiciones legales que les brinden apoyo legal, al igual que futuros estudios para validar y profundizar las competencias mapeadas.

Objective: to map the competencies of aerospace nurses in disaster situations. Method: a scoping review following the steps recommended by the JBI and the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) checklist. The review was conducted in three phases by two independent reviewers, with blinding, and supported by a third reviewer to resolve disagreements. Selection was based on the analysis of titles, descriptors, and abstracts, with specific eligibility criteria, followed by the full-text reading. At the end of the selection phase, 37 publications were included. Results: the results highlighted the need for the development of technical skills, knowledge of flight physiology, familiarity with aeronautical emergency procedures, communication skills, leadership, and responsibilities in aircraft preparation. During transport, nurses perform patient history taking (anamnesis), physical examinations, patient monitoring, clinical procedures, and manage in-flight complications. After the flight, they conduct documentation, develop procedures, sanitize clinical equipment, and replenish consumable materials. Conclusion: given the complexity of aerospace nursing practices in disaster situations, it is essential for professionals to develop competencies to ensure safe and effective care. There is a need to develop technologies, regulatory frameworks, and legal provisions for legal support, as well as future studies to validate and deepen the mapped competencies.
