Hemifacial Spasm: History, Epidemiology, Etiology, Pathogenesis and Anatomy – Problems of the Current State (Part I)

Abstract

Гемифациальный спазм (ГФС) – заболевание, проявляющееся безболезненными пароксизмальными непроизвольными односторонними тоническими или клоническими сокращениями лицевой мускулатуры, иннервируемой ипсилатеральным лицевым нервом. Частота встречаемости ГФС составляет примерно 11 на 100 000 населения, при этом заболевание манифестируется в среднем в районе третьего десятка жизни пациента. Гемифациальный спазм подразделяется на первичный, когда наличие спазмов обусловлено нейроваскулярным конфликтом в области лицевого нерва, и вторичный, когда наличие фасцикуляций лицевой мускулатуры обусловлено другими причинами. Первичный гемифациальный спазм чаще всего возникает из-за сдавления лицевого нерва артерией (передней нижней мозжечковой, задней нижней мозжечковой, позвоночной), реже венозными сосудами в области выхода нерва из ствола головного мозга. Учитывая распространенность гемифациального спазма, поражение преимущественно пациентов молодого возраста, инвалидизирующий характер заболевания, изучение этиологии, патогенеза, анатомических осбенностей первичного ГФС является актуальной проблемой современной нейрохирургии. Hemifacial spasm (HFS) is a disease manifested by painless paroxysmal involuntary unilateral tonic or clonic contractions of the facial muscles innervated by the ipsilateral facial nerve. The incidence of HFS is approximately 11 per 100,000 population, and the disease manifests itself on average around the third decade of the patient’s life. Hemifacial spasm is divided into primary – when the presence of spasms is due to a neurovascular conflict in the area of the facial nerve and secondary – when the presence of fasciculations of the facial muscles is due to other reasons. Primary hemifacial spasm most often occurs due to compression of the facial nerve by an artery (anterior inferior cerebellar, posterior inferior cerebellar, vertebral artery), less often – by venous vessels in the area where the nerve exits the brain stem. Considering the prevalence of hemifacial spasm, the affection of predominantly young patients, the disabling nature of the disease, the study of the etiology, pathogenesis, and anatomical features of primary HFS is an urgent problem of modern neurosurgery.
