Hemostastic embolization in oncology

Abstract: Several mechanisms predispose to bleeding in neoplastic disease. This is all the more serious as it often occurs on a background of medically vulnerable patients and the magnitude of the bleed may lead to hemorrhagic shock or acute respiratory distress as a result of hemoptysis. It often carries a poor prognosis, even if the acute episode has been controlled, as bleeding due to rupture of a tumor often indicates an advanced stage of the disease, and also because tumor rupture carries a risk of metastatic sprea… Show more

“…TAE is today recommended in a variety of haemorrhagic conditions such as trauma, haemoptysis, tumors, etc. (Coccolini et al 2017;Spiliopoulos et al 2018;Chun et al 2010;Revel-Mouroz et al 2015;Chakraverty et al 2012). Moreover, the continuously increasing population age, as well as the widespread use of more potent antithrombotic therapy has increased reports of severe spontaneous bleeding, while therapeutic anticoagulation at the time of bleeding, aggravates the severity of the event by triggering hemodynamic instability (Spiliopoulos et al 2019).…”

“…1 Massive hemoptysis can be life-threatening, with a mortality rate of more than 50% without adequate bleeding control. [2][3][4][5] Ninety percent of the source of massive hemoptysis comes from the bronchial circulation and 5% from the pulmonary circulation.…”

“…[1][2][3] Hemoptysis occurs in 30% of patients with lung carcinoma, with 10% of them experiencing massive bleeding. [2][3][4][5][6][7] The severity of hemoptysis depends on several factors, such as history of disease, coagulation, and hemorrhagic shock. 4 Bronchial Artery Embolization (BAE) is a diagnostic and therapeutic procedure in interventional radiology that has an important role in cases of massive hemoptysis.…”

“…Similarly, embolization can be successfully used in ruptured HCCs, retroperitoneal tumors such as angiomyolipomas and renal cell carcinomas, and severe hemorrhagic cystitis (cyclophosphamide, RT induced) refractory to conservative management. [3435]…”
