Heparin-Induced Thrombocytopenia

Abstract: Heparin-induced thrombocytopenia (HIT) is a prothrombotic disorder caused by antibodies that recognize complexes of platelet factor 4 (PF4) and heparin. HIT is frequently considered in the differential diagnosis of thrombocytopenia occurring in patients on heparin therapy. HIT is a challenging diagnosis because of routine heparin use in hospitalized patients, the common occurrence of thrombocytopenia, and high rates of anti-PF4/heparin seroconversions in patients treated with heparin. This chapter will summari… Show more

“…It may be safe to use heparin after this window, but expert opinion suggests the use of alternative agents in patients with a history of HIT, even if remote. 24 Low-molecular-weight heparin and UFH are the most widely studied agents for perioperative thromboprophylaxis. For surgical patients, evidence for DOAC use is limited to hip and knee arthroplasty.…”

“…The patient's blood platelet count is closely followed, as heparin-associated thrombocytopenia (HIT) can occur on heparin. 12 HIT always needs to be considered in a patient on heparin (unfractionated heparin or low molecular weight heparin) who develops a new thrombotic event or has a platelet count decrease of Ͼ30% from baseline or below 100 000ϫ10 6 /mL. It is a potentially devastating complication of unfractionated heparin, less commonly of low molecular weight heparin therapy.…”
