Hepatic Abscess in a Returning Traveler with Crohn’s Disease: Differentiating Amebic from Pyogenic Liver Abscess

Abstract: Liver abscess is a rare but serious complication of Crohn's disease. Patients with Crohn's disease are at risk for pyogenic liver abscesses due to immunosuppressive therapy, fistulous disease, and intraabdominal abscesses. Inflammatory bowel disease patients are also known to have a greater prevalence of amebiasis compared to the rest of the population; however, a higher incidence of amebic liver abscess has not been reported. We describe a case of a liver abscess in a patient with Crohn's disease that was ini… Show more

“…Based on the causative organism, hepatic abscess can be broadly classified into pyogenic and amebic entities. Although the 2 categories have some overlapping features, it is crucial to differentiate between them as the management and prognosis markedly differ [ 8 ]. Pyogenic forms, for example, tend to present as multiple lesions and have a polymicrobial origin [ 2 , 8 ].…”

Escherichia coli , Streptococcus species and Klebsiella pneumoniae are among the predominant causative pathogens of PLA, with the latter being the most aggressive agent [ 5 , 9 ].…”

“…The hepatic abscess (HA) is an infectious disease characterized by a capsule collection with a suppurative content in the context of the hepatic parenchyma, whose nature may be of bacterial, fungal and/or parasitic origin [1,2]. It represents an extremely severe and potentially fatal condition with an incidence rate ranging from 6 to 15% [3][4][5].…”

“…[ 1 ] Majority of the cases in developing countries are of amebic etiology, whereas more people suffer from pyogenic liver abscess (PLA) in developed parts of the world. [ 2 ] Entamoeba histolytica , the causative agent of amebiasis, is a protozoan parasite of humans that is estimated to cause amebic liver abscess (ALA) in around 50 million people across the globe. [ 3 ] It is the third leading parasitic cause of death after malaria and schistosomiasis, with an estimated mortality of 100,000 people per year.…”
