Hepatic PEComa: a potential pitfall in the evaluation of hepatic neoplasms

Abstract: Perivascular epithelioid cell tumour (PEComa) of the liver is very uncommon and may be overlooked in the clinical and histological differential diagnosis of a liver tumour. We report the case of an incidentally discovered liver mass suspicious for hepatocellular carcinoma, which on biopsy was suggestive of a pseudocyst but after resection was found to be hepatic PEComa with some of the usual characteristics of this neoplasm as well as several less familiar features. We have also reviewed cases of hepatic PECom… Show more

“…Patients admitted to both departments had no established diagnosis. In most cases, the lesions resembled HCC on CT and MRI [8,13]. All patients treated in our center developed hepatic lesions even though they had healthy livers, did not have a history of hepatitis, and did not have any factors that damage the parenchyma.…”

“…Usually, these are single lesions; however, multifocality was also reported more frequently in cases involving malignant lesions [12]. Differential diagnoses include other liver tumors, such as hepatocellular carcinoma (HCC), hemangiomas, focal nodular hyperplasia, and liver adenomas [8,13]. In liver ultrasound examination, computed tomography (CT), and magnetic resonance imaging (MRI), the tumors are often misdiagnosed as HCC.…”

“…The diagnosis of hepatic PEComa mainly relies on the unique morphology of tumour cells and presence of specific immunological markers including HMB-45, SMA and Melan-A. Fine-needle biopsy is recommended for the diagnosis of hepatic PEComa, which effectively avoids the substantial trauma caused by surgery [22] . In addition, hepatic PEComas must be differentiated from other tumours.…”

“…In the present study, hepatic PEComas presented as a heterogeneous hypointensity on T1WI and hyperintensity on T2WI. However, as the cystic components decrease and the substantial parts increase, the tumour will be hyperintense on T1WI [22]. Hepatic PEComas are viewed as high signal masses on DWI, whereas on ADC, they may present the results.…”
