Hepatic subcapsular flow: An early marker in diagnosing biliary atresia

Abstract: We report an interesting sign in the sonological evaluation of neonatal cholestasis, which is hepatic subcapsular flow. Hepatic subcapsular flow is an early and useful marker in diagnosing biliary atresia.

“…A similar phenomenon was also observed in Lee’s[ 8 ] and Ramesh’s[ 27 ] studies. These authors reported that patients with BA who had hepatic subcapsular flow on color Doppler US images also had subcapsular telangiectatic vessels on the liver surface at the time of the Kasai procedure.…”

“…El-Guindi et al[44] in a recent study reported the superiority of demonstration of hepatic subcapsular flow over the other sonographic parameters such as triangular cord sign, GB contractility, GB size and dimensions of hepatic artery. Even when the sonographic hallmark of triangular cord sign could not be satisfactorily demonstrated, presence of hepatic sub capsular flow can be of significant value in sonographic examination[45]. …”

“…However, IL-12p70 and IL-23 together can drive effector function in memory CD4 + cells and this underpins the pathogenesis of many human inflammatory conditions ( 26 ). IL-23 drives effector function of Th17 cells characterized by production of IL-17A, IL-17F, IL-21, and IL-22 ( 26 ) and the presence of IL-12 reprograms memory Th17 cells into a “Th17/Th1” phenotype that are more aggressive and pathogenic, favoring autoimmune disease progression ( 11 , 27 30 ).…”
