AraştırmaÖZET Amaç: Hepatit A infeksiyonları subklinik seyretmesi nedeniyle az oranda klinik olarak tanımlanabilmektedir. Bu durumda insidansı belirlemenin en güzel yolu farklı coğrafik bölgelerde HAV prevalansını saptamaktır. Biz de bu çalışma ile Muş bölgesinde Hepatit A görülme sıklığını ve HAV seropozitivitesini belirlemeyi amaçladık. Gereç ve Yöntem: Ocak-Ekim 2014 tarihleri arasında hastanemize başvuran 0-14 yaş arası 369, 14 yaş üstü 1063 hasta retrospektif olarak incelendi. Bulgular: Sonuçlar, hastalığın yaş ile ilişkili olarak görülme sıklığı ve ilerleyen yaşlarda kazanılan immunite açısından analiz edildi. On dört yaşta dâhil olmak üzere, sonrasında IgM pozitifliğine hiç rastlanmazken; çocuklarda HAV IgM %20.48 (68/369) pozitif olup, en sık 0-6 (41/68) yaş arasında görüldü. Erişkin grupta IgG %95.86 (1019/1063) pozitifti. Sonuçlar popülasyonun büyük çoğunluğunun çocuk yaşta infeksiyon ile karşılaştığını, HAV infeksiyonlarının bu bölgede yüksek oranda görüldüğünü ve yaş ile beraber HAV seropozitivitesinin arttığını gösterdi. Tartışma: Özellikle sosyoekonomik düzeyin düşüklüğü, kalabalık aile ortamları, kültürel yaşam alışkanlıkları, kırsal nüfusun kent nüfusundan fazla oluşu, su arıtma sistemlerinin yetersizliği ve atıkların iyi kontrol edilemiyor oluşu Hepatit A enfeksiyonlarını Muş ili için önemli bir sorun haline getirmektedir. Bu nedenle yaşayan halkın bu enfeksiyon hakkında bilgilendirilmesi gerektiğini düşünmekteyiz. Anahtar kelimeler: Muş, Hepatit A, çocuk, erişkin SUMMARY Prevalence of Hepatitis A in Patients Admitted to Muş State Hospital, TurkeyObjective: Hepatitis A virus (HAV) infections are usually less recognized due to their characteristic subclinical presentation especially in children. The best approach is to determine the prevalence in different geographical regions. In this study we aimed to determine the seroprevalence of HAV seropositivity in Muş region, Turkey. Material and Method: Hepatitis A seropositivity was investigated retrospectively in 369 pediatric (0-14 years old) and 1063 adults patients between January 2014 and October 2014.Results: Results were analyzed in terms of the prevalence of hepatitis A virus in relation to age and also the rate of acquired immunity in adulthood. Hepatitis A virus IgM positivity was not observed in the adult group. HAV IgM positivity was 20.48% (68/369) in children. HAV IgG positivity was 95.86% (1019/1063) in adult group. The results indicated that the majority of the population encountered HAV during childhood.Conclusion: HAV infections were highly prevalent in the study area and the seropositivity of HAV increased with age. Higher prevalence of HAV infection in Muş area was possibly attributed to the low socioeconomical status, crowded family life, cultural and lifestyle habits, lack of proper sanitary systems and living more in the rural area than the urban area in Muş province.

Background The incidence of Hepatitis A is increasing worldwide. Yearly classification of Hepatitis A Seroprevalence for different times in different regions of Türkiye allows indirect measurement of year-specific incidence rates of HAV infection and can be considered the best way to define Hepatitis A status. This study aimed to examine the change in the incidence of hepatitis A by systematically evaluating the available data on the seroprevalence of anti-HAV antibodies in the Turkish population. Methods Studies published between First January, 2000- 31 December 2023 that meet the inclusion criteria searched between 16.09.2023 and 31.01.2024 in nine databases (MEDLINE, Web of Science, PubMed, ScienceDirect, Scopus, Eric, CINAHL Complate, TR DİZİN, TÜBİTAK Ulakbim). Independently by two reviewers evaluated all titles and abstracts with consensus-based decision making. The Joanna Briggs Institution (JBI) Analytical Cross Sectional Studies and Prevelance Studies list were used in this study. Pooled prevalence was calculated using 95% confidence intervals. Heterogeneity between studies was assessed by Cochrane’s Q and I2. The random effect model was selected by Cochrane’s Q and I2. Funnel plots were used for publication bias. The data were analyzed via Jamovi 1.2.22. Results Data were extracted from 63 studies. Pooled prevalence was calculated using 95% confidence intervals. Funnel plots were used for publication bias. In this meta-analysis, data were obtained from all geographical regions of Türkiye, and the general prevalence of Hepatitis A in the population was found to be 53% [95% CI 0.47, 0.59; I2 = 99.94%]. In subgroup analysis conducted by year, the prevalence of hepatitis A was 45% [95% CI 0.22, 0.67; I2 = 99.8%] between 2002 and 2006, 52% [95% CI 0.39, 0.65; I2 = 99.54%] between 2007 and 2011, 60% [95% CI 0.49, 0.70; I2 = 99.9%] between 2012 and 2016, and 51% [95% CI 0.41, 0.61; I2 = 99.97%] as of 2017. Additionally, a systematic review revealed that vaccination, socioeconomic status, and sex may also affect HAV seroprevalence. Conclusions HAV seroprevalence in Türkiye; It was observed that it increased between 2002 and 2016 and decreased until today as of 2017. This systematic review provide a comprehensive overview of HAV virus epidemiology and identify key knowledge gaps, contributing crucial information for influencing factors.
