Herpes simplex virus type 1 in Europe: systematic review, meta-analyses and meta-regressions

Abstract: ObjectiveTo describe the epidemiology of herpes simplex virus type 1 (HSV-1) in Europe.MethodsWe systematically reviewed HSV-1 related publications, conducted various meta-analyses and meta-regressions, assessed pooled mean seroprevalence, and estimated pooled mean proportions of HSV-1 viral detection in clinically diagnosed genital ulcer disease (GUD) and in genital herpes.ResultsWe extracted, from 142 relevant records, 179 overall (622 stratified) seroprevalence measures, 4 overall proportions of HSV-1 in GU… Show more

“…HSV-2 infection (as opposed to HSV-1 infection) proved to be the etiological cause of 76% of genital herpes cases, lower than in Africa at 97% 24 and in LAC at 91%. 39 This finding is consistent with the finding that HSV-1 infection now plays a growing role in genital herpes in Asia, 25 just as in North America 40 , 54 and Europe, [55] , [56] , [57] , [58] , [59] but differs from the pattern found in Africa, 60 LAC, 61 and possibly the Middle East and North Africa, 62 where sexual transmission of HSV-1 infection is still less pervasive. Curiously, HSV-2 seems more significant in genital herpes among men than women ( Table 4 ), a finding that may reflect the relative ease of clinically diagnosing this condition in men compared to women.…”

“…(80, 81) HSV-2 infection (as opposed to HSV-1 infection) was also found to account for nearly all cases of genital herpes (>97%; Table 5). This finding is presumably due to the nature of HSV-1 being widely acquired in childhood in SSA through oral transmission,(33) and distinguishes this region from other global regions where there is an increasing role for HSV-1 in genital herpes,(3336) with some countries already observing HSV-1 as the cause of a large proportion of cases of genital herpes. “…30 While this finding is consistent with what is observed in sub-Saharan Africa and possibly the Middle East and North Africa, [77][78][79] it contrasts with what is observed in North America, Europe, and Asia, where the role of HSV-1 in genital herpes has been increasing, and in some settings and populations even reaching the point of being the leading cause of this disease outcome. [80][81][82][83][84][85][86][87][88] This study has limitations. HSV-2 epidemiological data were mainly available for the large countries of Latin America and the Caribbean region that constitute most of its population, but there were no data available for 27 out of the 47 (mostly small) countries constituting this part of the world.…”
