High dose androgen therapy in male pseudohermaphroditism due to 5 alpha-reductase deficiency and disorders of the androgen receptor.

Abstract: Abstract. We describe the clinical and biochemical features of six men with male pseudohermaphroditism due to androgen resistance. Each of the subjects had male-gender behavior but incomplete virilization. The underlying defects in androgen metabolism were defined by studies of the 5a-reductase enzyme and the androgen receptor in fibroblasts cultured from biopsies of genital skin. Four of the six have 5a-reductase deficiency, and two have defects of the androgen receptor (the Reifenstein syndrome). The response… Show more

“…First, individuals with a complete deletion of the type 2 gene (12) can have serum dihydrotestosterone levels within the normal range (5). Second, the administration oftestosterone to an individual with a splicing mutation in the type 2 gene results in normal serum dihydrotestosterone levels (31 ). These results also suggest that the effective treatment of diseases such as BPH that are caused in part by the action of dihydrotestosterone will require pharmacological inhibition of bothSa-reductase isozymes.…”

“…Apart from one successful experience with mesterolone (1a-methylandrostan-17β-ol-3-one) in promoting spermatogenesis and fertility twice in a man with MAIS, 98 experience with long-term natural androgen pharmacotherapy is meager and its value unclear. 44,[99][100][101] …”
