High‐dose steroid therapy for acute respiratory distress syndrome lacking common risk factors: predictors of outcome

Abstract: AimAcute respiratory distress syndrome (ARDS) is a life‐threatening lung disease that usually occurs in patients with the underling risk factors that triggers lung inflammation. We sometimes encounter patients with ARDS lacking common risk factors. Recent studies have indicated the effectiveness of corticosteroids for this cohort. However, the characteristics of survivors with ARDS who lack common risk factors, and who received high‐dose methylprednisolone pulse therapy (MPPT), are not known.MethodsWe undertoo… Show more

“…In a clinical setting, MP is predominantly administered intravenously or orally ( Kinoshita et al, 2017 ; Meduri et al, 2018 ), while other corticosteroid drugs, such as budesonide and formoterol, have proven clinical benefits when inhaled ( Barnes, 2011 ). However, the aim of this study was not to evaluate the action of MP by itself but rather to show the responsiveness of the model to corticosteroid treatment in vitro .…”

“…Furthermore, intravenous high-dose methylprednisolone pulse therapy (IHDMPPT) at 1 g per person for 3 consecutive days, with or without subsequent tapered lower dose steroid therapy, is frequently prescribed for ARDS treatment by physicians in Japan [ 14 ].…”

“…As cytokine storm is relatively common in severe cases and often leads to exacerbation [11], several anti-in ammation treatments are proposed, including non-steroidal anti-in ammatory drugs, and glucocorticoids, chloroquine/hydroxychloroquine, immunosuppressant, in ammatory cytokines antagonists [12]. Concerning the widespread use of corticosteroids as a therapy for ARDS [13]and proof of e cacy to treat SARS, which could signi cantly decrease the mortality of severe SARS patients, we aimed to evaluate the e cacy of methylprednisolone pulse on intermediate, and severe COVID-19 patients admitted to Intensive Care Unit (ICU).…”
