High‐dose therapy with fluconazole ≥ 800 mg day‐1

Abstract: Summary. Fluconazole dosages greater than 800 mg day‐1 have been reported in about 900 patients treated for candidemia, oropharyngeal candidiasis and cryptococcal meningitis in HIV‐infected patients, and for initial therapy of endemic mycoses*. In patients with life‐threatening infections caused by Candida spp., Cryptococcus neoformans and Coccidioides immitis, results of a limited number of dose‐finding trials with non‐neutropenic and HIV‐infected patients show dose‐dependent responses. These study results i… Show more

“…The combination of amphotericin B plus fluconazole may ameliorate some of these difficulties. While both fluconazole and flucytosine are excreted renally and can be expected to accumulate as renal function deteriorates during amphotericin B use, the safety profile of fluconazole is far superior to that of flucytosine, even when high doses of fluconazole are used (11). Thus, if renal function deteriorates as a result of amphotericin B use, the increasing fluconazole levels are not likely to lead to adverse clinical effects.…”

“…Acinetobacter baumannii meningitis has been treated successfully with 2 g ampicillin plus 1 g sulbactam every 3 h without severe adverse drug effects (33). The daily dose of fluconazole can be increased up to 1,600 mg with tolerable side effects (44,58).…”

“…Of our isolates, 82.9% (174 of 210) fell into this category. The other isolates that were not susceptible by the disk test should undergo more-precise procedures such as NCCLS macrodilution tests or microdilution tests (5). We conclude that fluconazole disk tests on an MH agar plate containing 2% glucose and 5 g of methylene blue/ml at 24 h can be used as a routine screening procedure for susceptibility of Candida species to fluconazole in clinical laboratories.…”
