Immune checkpoints are crucial for the maintenance of self-tolerance and for the modulation of immune responses in order to minimize tissue damage. Tumor cells take advantage of these mechanisms to evade immune recognition. A significant proportion of tumors, including breast cancers, can express co-inhibitory molecules that are important formediating the escape from T cell-mediated immune surveillance. The interaction of inhibitory receptors with their ligands can be blocked by specific molecules. Monoclonal antibodies (mAbs) directed against the cytotoxic T lymphocyte-associated antigen-4 (CTLA4) and, more recently, against the programmed cell death protein 1 (PD1), have been approved for the therapy of melanoma (anti-CTLA4 and anti-PD1 mAbs) and non-small cell lung cancer (anti-PD1 mAbs). Moreover, inhibition of PD1 signaling has shown extremely promising signs of activity in breast cancer. An increasing number of molecules directed against other immune checkpoints are currently under clinical development. In this review, we summarize the evidence supporting the implementation of checkpoint inhibition in breast cancer by reviewing in detail data on PD-L1 expression and its regulation. In addition, opportunities to boost anti-tumor immunity in breast cancer with checkpoint inhibitor-based immunotherapies alone and in combination with other treatment options will be discussed.

Breast cancer is the most prevalent breast cancer in women. Despite our deeper understanding of mammary oncogenesis, 20% of breast cancer patients develop metastasis and die. There is a dire need for novel therapies especially for more aggressive subtypes like triple negative (TNBC) and HER2 positive breast cancers. For long, breast cancer was thought to be less immunogenic in nature compared to other solid malignancies like melanoma and non-small cell lung cancer, both later cancers yielding great results with immunotherapy. As we learn more about the role of tumor microenvironment in tumor occurrence, progression and response to therapy, there is growing evidence that TNBC is more immunogenic in nature compared to other breast cancers, which encouraged researchers to investigate its response to immune check point inhibitors. TNBC is characterized by a high level of immune cell infiltration including tumor infiltrating lymphocytes (TILs) which have been associated with better outcomes. The prognostic values of TILs and PD-L1 expression on tumor cells have been an area of interest and remain uncertain to date. The hope is that future trials will unravel more answers about the complex interaction between TILs and tumor cells in TNBC and help develop biomarkers to identify responders to checkpoint inhibitors. In this review, we discuss the immunogenic nature of TNBC, the clinical relevance of TILs and PD-L1 expression and the promising results of early phase trials of checkpoint blockade.

Advances in research have transformed our understanding of breast cancers and have altered the daily practice of pathology. Theranostic evaluations performed by pathologists are now critical in triaging the patients into appropriate treatment groups, as are new guidelines that were recently established for the evaluation of HER2/neu gene amplification. Emerging molecular classifications of breast cancers bring novel perspectives to the assessment of individual cases, and opportunities for better treatments. Molecular studies have particularly shed light on distinct biological subsets of triple-negative breast cancers, for which new targeted therapies are being developed. The prognostic and therapeutic utility of new histopathologic parameters, such as tumor-infiltrating lymphocytes, are also being elucidated, and new protocols have been devised for the pathologic evaluation of breast specimens that have undergone neoadjuvant treatment. Novel clinical practices, such as radioactive seed localization, also affect the way breast specimens are processed and evaluated. In this brief review, we highlight the developments that are most relevant to pathology and are changing or could potentially impact our daily practice.
