High prevalence of celiac sprue-like HLA-DQ genes and enteropathy in patients with the microscopic colitis syndrome

Abstract: A shared set of predisposing HLA-DQ genes account for the epidemiological overlap of celiac sprue and microscopic colitis. Mild to moderate mononuclear cell inflammation of the small intestine, often accompanied by partial or subtotal villous atrophy, is frequent in patients with the microscopic colitis syndrome. Although further studies will be necessary to determine if this enteropathy is induced by dietary gluten, we speculate that the small intestinal but not colonic histopathology in patients with microsc… Show more

“…Human leukocyte antigen (HLA) studies have shown an association between MC and HLA-DQ2 or DQ1/3, and recently an association has reported between MC and HLA-DR3-DQ2 haplotype and with TNF2 allele carriage, irrespective of the presence of concomitant celiac disease [50,51] . Variants of the MMP-9 gene have been reported to be associated with CC [52] .…”

“…An interesting point of view is the possibility of an abnormal reaction to a luminal antigen. This is supported by the reported coincidence of coeliac disease (CD) and LC and the detection of anti-gliadin, anti-endomysial and anti-tissue transglutaminase antibodies in up to 17% of the patients with LC [8][9][10].…”

“…Reports of familial clustering of MC [79], associations with certain HLA haplotypes (HLA-DQ2 or DQ1,3) [80], and polymorphisms in TNF-a gene [81] suggest a possible underlying genetic component. The strong association with autoimmune conditions suggests an underlying immune dysregulation.…”
