Hip involvement in ankylosing spondylitis: epidemiology and risk factors associated with hip replacement surgery

Abstract: Hip involvement is commonly recognized by rheumatologists in AS patients, and involves about one out of the three to four patients with AS and is associated with impaired functioning reflected by higher overall BASFI scores. Early onset of disease, axial and enthesial disease are associated with the hip replacement surgery in AS.

“…Hip pain can be a result of inflammation of the subchondral bone marrow. This inflammation is different from classical spine problems in that it is not formation of new bone, but an erosive process that can often lead to hip replacement (6) . Furthermore, AS can affect organs such as the eyes, heart, and kidneys; lung capacity can also be restricted because of the effects of AS on the ribcage.…”

“…Hip involvement in patients with SpA is a common, disabling problem and is believed to be predictive of worse outcomes. 7,8 Previous studies indicate that synovitis is the most common MRI finding of hip involvement in patients with SpA. Synovitis is often the only sign observed at the early stages of hip involvement.…”

“…Hip involvement is another important feature of axial SpA and is present in approximately one-third of the patients (8). Hip involvement is associated with impaired functioning; it may increase the burden of disease and influence the prognosis (8).…”
