Hip-Sparing Equalization Procedures for Leg-Length Discrepancy after Total Hip Arthroplasty: A Retrospective Case Series

Abstract: Background Leg-length discrepancy (LLD) after primary THA is not uncommon. Little is known, however, about the role of hip-sparing procedures for equalization of LLD after THA. Questions/Purposes The aim of this study is to report our experiences with these techniques in patients presenting at one institution over a 10-year period. Methods We retrospectively reviewed records at one institution to find patients who had sought surgical treatment for LLD after THA between January 2007 and August 2017. Patient… Show more

“…LLD can be difficult to avoid in hip arthroplasty, as the surgeon must ensure adequate soft tissue tension (a task that may result in lengthening), while avoiding the stretching of the sciatic nerve (a task that may result in shortening). 32 LLE can nonetheless be achieved through attention to offset, socket position, and neck length. 14 Generally, 0-5 mm of leg lengthening should be considered adequate for soft tissue tensioning.…”

“…Total hip arthroplasty does not preclude lengthening. 27 In addition, growth modulation by means of epiphysiodesis or hemiepiphysiodesis for distal femoral valgus may not be possible in later presenting patients. In that instance, an acute shortening or wedge osteotomy with internal fixation may be necessary.…”
