Histologic Factors Associated With Need for Surgery in Patients With Pedunculated T1 Colorectal Carcinomas

Abstract: In a cohort-nested matched case-control study of 708 patients with pedunculated T1 colorectal carcinomas, we developed a model based on histologic features of tumors that identifies patients who require surgery (due to high risk of metastasis) with greater accuracy than previous models. Our model might be used to identify patients most likely to benefit from adjuvant surgery.

“…Lastly, Backes et al assessed the utility of incorporating PDC into a model aimed at assessing the need for surgical resection in pedunculated pT1 cancers. 27 They matched 37 cases with local recurrence, distant metastases and/or lymph node metastases, with 111 cases without, and compared rates of various histological parameters within the two groups. They found a significantly higher rate of high-grade PDC (defined as G2/G3 PDC ) in cases (62%) compared with controls (24%).…”

“…5 Another study incorporated the presence of either Bd2/3 or G2/3 PDC as a single risk factor into a predictive model to determine the risk of lymph node metastasis in pedunculated pT1 cancers. 27 A handful of studies have compared protein expression and molecular profiles in PDC and TB, with most finding significant overlap between these two features. Hong et al evaluated the expression of beta-catenin, Ki-67, E-cadherin, mucin 1 (MUC1) and epithelial membrane antigen (EMA) in PDC, TB and background tumour.…”

“…13,34,35 On the other hand, the number as well as characteristics of pathological risk factors may be associated with regional lymph node metastasis risk. Backes et al 36 developed the LASSO (Least Absolute Shrinkage and Selection Operator) model to calculate the probability of pedunculated T1 cancer metastasis by inputting the presence or absence of various pathologic risk factors. Our study also con- Values are presented as number (%).…”

“…The least absolute shrinkage and selection operator (LASSO) method was originally designed for regression analysis [19]. Lately, it has been applied to many fields, including the construction of prognostic models for various cancers [20,21,22,23].…”
