HIV decline in Zimbabwe due to reductions in risky sex? Evidence from a comprehensive epidemiological review

Abstract: Background Recent data from antenatal clinic (ANC) surveillance and general population surveys suggest substantial declines in human immunodeficiency virus (HIV) prevalence in Zimbabwe. We assessed the contributions of rising mortality, falling HIV incidence and sexual behaviour change to the decline in HIV prevalence. Methods Comprehensive review and secondary analysis of national and local sources on trends in HIV prevalence, HIV incidence, mortality and sexual behaviour covering the period 1985–2007.Results … Show more

“…This is affordable for most developing countries without external donor support. Zimbabwe reduced HIV prevalence by half between 1997 and 2007, largely by promoting positive changes in sexual behaviours (Gregson et al, 2010;Hallett et al, 2006). This drop occurred despite political and civil unrest, agricultural land reform, and currency hyperinflation that devalued their dollar by more than 200 million percent, placing Zimbabwe at the world's lowest ranking on the Human Development Index.…”

“…A recent meta-analysis of 42 studies, covering 67 behavioral interventions, indicated an associated decrease in sexual risk-taking resulting in increased condom use, and fewer STIs, including HIV [23]. Many countries, including Kenya, Malawi, South Africa, Tanzania, Trinidad, Zambia and Zimbabwe, are reporting favorable results from behavioral interventions [24][25][26][27]. Comprehensive sexuality education is indispensable for behavior change and has been demonstrated to increase knowledge and decrease risktaking [28].…”

“…These changes brought about by ART resulted in savings of over half a million dollars (USD) in the two years, including US$287 000 saved due to reduced absenteeism. Manicaland is a rural province which had an adult HIV prevalence rate of about 20% (Gregson, et al, 2010). The interviews sought to elicit descriptions of how employment helps or hinders adherence, and how workplaces provide support or present challenges.…”
