Psoriasis is a common T-cell-mediated skin disease with 2-3% prevalence worldwide. Psoriasis is considered to be an autoimmune disease, but the precise nature of the autoantigens triggering T-cell activation remains poorly understood. Here we find that two-thirds of patients with moderate-to-severe plaque psoriasis harbour CD4 þ and/or CD8 þ T cells specific for LL37, an antimicrobial peptide (AMP) overexpressed in psoriatic skin and reported to trigger activation of innate immune cells. LL37-specific T cells produce IFN-g, and CD4 þ T cells also produce Th17 cytokines. LL37-specific T cells can infiltrate lesional skin and may be tracked in patients blood by tetramers staining. Presence of circulating LL37-specific T cells correlates significantly with disease activity, suggesting a contribution to disease pathogenesis. Thus, we uncover a role of LL37 as a T-cell autoantigen in psoriasis and provide evidence for a role of AMPs in both innate and adaptive immune cell activation.

StreSzczenieObecnie uważa się, że łuszczyca należy do kręgu schorzeń autoimmunologicznych, współistniejących z innymi chorobami z tej grupy. U pacjentów stwierdza się najczęściej łuszczycowe zapalenie stawów, reumatoidalne zapalenie stawów, choroby tkanki łącznej, stwardnienie rozsiane, autoimmunologiczne choroby tarczycy, celiakię, nieswoiste zapalenia jelit. Współwystępowanie tych zaburzeń może stanowić problem diagnostyczny i leczniczy (kontrowersje dotyczące stosowania kortykosteroidów). Wspólna patogeneza pozostaje nadal nie do końca wyjaśniona. Uważa się, że utrata immunotolerancji prowadzi do powstania m.in. autoreaktywnych limfocytów Th1 i Th17, które rozpoznają autoantygeny i prowadzą do ich niszczenia w danym narządzie docelowym. Pewne cechy mechanizmów immunologicznych obserwowanych w łuszczycy sugerują jej autoimmunologiczne tło. Należą do nich uwarunkowania genetyczne, np. PSORS 3 na chromosomie 4q predysponuje do celiakii, cukrzycy typu 1, choroby Gravesa-Basedowa i reumatoidalnego zapalenia stawów. Łuszczyca jest chorobą, w któ-rej decydującą rolę odgrywa aktywacja osi IL-12/Th1/IFN-γ i Th17/ IL-23. Prawdopodobnie IL-12 działa na naiwne limfocyty T i zapocząt-kowuje odpowiedź Th1, a IL-23 podtrzymuje reakcję zapalną mediowaną przez Th1, pobudza dojrzewanie i aktywność Th17 oraz wpływa na utrzymanie odpowiedniej puli komórek pamięci. Dochodzi również do zaburzenia funkcji limfocytów Treg, odpowiedzialnych za niszczenie limfocytów autoreaktywnych. Ponadto keratynocyty łuszczycowe mają zwiększoną oporność na apoptozę, której zadaniem jest eliminacja uszkodzonych komórek, aby nie zostały rozpoznane jako antygenowo obce. Badacze sugerują jednak, że początkowo następuje poliklonalna aktywacja limfocytów T wywołana przez superantygeny (m.in. paciorkowcowe białka M, peptydoglikan) lub uraz naskórka (objaw Köbne-ra), natomiast w późniejszej fazie autoreaktywne limfocyty T rozpoznają autoantygeny (keratyna 17, białka kapsydu L1 HPV 5, Pso p27) w naskórku, co prowadzi do reakcji autoimmunologicznej. Keratin 17: A Critical Player in the Pathogenesis of Psoriasis

Keratin 17 (K17) is an intermediate filament protein present in the basal cells of complex epithelia, such as nails, hair follicles, sebaceous glands, and eccrine sweat glands. Studies have shown that it is expressed aberrantly in the suprabasal keratinocytes of psoriatic lesions, compared to in normal epidermis. K17 is also closely associated with the immune system and plays an important role in the pathogenesis of psoriasis. In this review, we present our experimental findings concerning the role of K17 in psoriasis, and compare them to results published in the literature. Our results show that cytokines related to Th17 and IL-22-producing (where Th17 is T helper cells, type 17 and IL is interleukin) CD4(+) T cells, including IL-17A and IL-22, upregulate the expression of K17 in keratinocytes. In addition, K17 stimulates autoreactive T cells and promotes the production of psoriasis-associated cytokines. Our findings lend support to the hypothesis that a K17/T-cell/cytokine autoimmune loop is involved in the pathogenesis of psoriasis. We therefore review the current understanding of the K17 immunoregulation, including its expression and direct/indirect effects on immune responses. Pertinent strategies for the treatment of psoriasis are also discussed.
