Hormones regulating cardiovascular function in patients with severe congestive heart failure and their relation to mortality. CONSENSUS Trial Study Group.

Abstract: There is a varying hormonal activation in heart failure. To be able to evaluate this activation and relate it to prognosis, we took blood samples at baseline and after 6 weeks from 239 patients with severe heart failure (all in New York Heart Association class IV) randomized to additional treatment with enalapril or placebo. In this study (CONSENSUS), which has previously been reported, there was a significant reduction in mortality among patients treated with enalapril. The present data show in the placebo gr… Show more

“…Chronic activation of the renin‐angiotensin‐aldosterone system (RAAS) contributes to the development of congestive heart failure (CHF) and is associated with increased morbidity and mortality in human patients and dogs with cardiac disease 1, 2, 3. Angiotensin converting enzyme inhibitors (ACEI) are commonly used to suppress this system and are considered standard of care in dogs for treatment of CHF secondary to myxomatous mitral valve disease (MMVD) 4.…”

“…ACE inhibitors also prevent the remodeling of the myocardium after acute myocardial infarction. Continuously increased activation of renin-angiotensin-aldosteron and sympatho-adrenergic systems is clearly negatively related to prognosis in patients with congestive heart failure [18][19][20]. For all these positive properties ACE inhibitors are recommended both by the US and European guidelines [21,22] as first line drugs for congestive heart failure.…”

“…In the CONSENSUS I study 94 , increased levels of angiotensin II and aldosterone, in addition to norepinephrine and ANF, had a significant correlation with mortality. Angiotensin II had a stronger correlation than norepinephrine, the opposite of what had been found in previous studies.…”
