Prehospital care has historically provided medical care with the aim of saving lives and preventing disability. Prehospital services that are accommodated by ambulances have complex and unpredictable care arrangements, so ambulance personnel, namely nurses, must prepare themselves to deal with all situations and conditions. This study aims to describe the competencies possessed by nurses while on duty in prehospital ambulances. This research is a literature study from several databases, and we obtained 20 articles for analysis. The results of the article analysis show that nurses who work in prehospital ambulances must have the competence to be able to save patients' lives, which is formed from knowledge, attitudes, and skills. Knowledge that must be possessed by ambulance nurses includes assessment, triage, and situation analysis. The positive attitudes of ambulance nurses include decision-making, communication, collaboration, caring, ethics, and culture. The skills possessed by ambulance nurses are BHD, intubation, examination of vital signs, interpretation of supporting examinations, and drug management. Ambulance nurses can improve their emergency care competencies through continuous education and training.

Background: An under-developed and fragmented prehospital Emergency Medical Services (EMS) system is a major obstacle to the timely care of emergency patients. Insufficient emphasis on prehospital emergency systems in low- and middle-income countries (LMICs) currently causes a substantial number of avoidable deaths from time-sensitive illnesses, highlighting a critical need for improved prehospital emergency care systems. Therefore, this systematic review aimed to assess the prehospital emergency care services across LMICs. Methods: This systematic review used four electronic databases, namely: PubMed/MEDLINE, CINAHL, EMBASE, and SCOPUS, to search for published reports on prehospital emergency medical care in LMICs. Only peer-reviewed studies published in English language from January 1, 2010 through November 1, 2022 were included in the review. The Newcastle–Ottawa Scale (NOS) and Critical Appraisal Skills Programme (CASP) checklist were used to assess the methodological quality of the included studies. Further, the protocol of this systematic review has been registered on the International Prospective Register of Systematic Reviews (PROSPERO) database (Ref: CRD42022371936) and has been conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Results: Of the 4,909 identified studies, a total of 87 studies met the inclusion criteria and were therefore included in the review. Prehospital emergency care structure, transport care, prehospital times, health outcomes, quality of information exchange, and patient satisfaction were the most reported outcomes in the considered studies. Conclusions: The prehospital care system in LMICs is fragmented and uncoordinated, lacking trained medical personnel and first responders, inadequate basic materials, and substandard infrastructure.

Introduction Most patients are quickly transferred to the hospitals by ambulances worldwide. Hence, it is imperative that ambulance should be well equipped with all emergency equipment and well-trained staff to provide cardiopulmonary resuscitation (CPR) or basic life support (BLS) to victim in pre-hospital environment, if required. Purpose To assess knowledge regarding CPR of ambulance staff, equipped status of ambulances, and their utilization during CPR or first aid. Methods Total 400 ambulance staff were purposively recruited after obtaining informed written consent. They were interviewed as per validated interview schedule regarding CPR knowledge and utilization of equipment during CPR or first aid. Using an observation checklist for assessment of available equipment for CPR and first aid in the ambulance was checked. Results Mean age of participants was 35.57 ± 7.63 years. All were males. Among them 63.75% were posted in private ambulance, and 73.50% ambulances had solo staff. Most (98.25%) ambulances were equipped with oxygen cylinder and only 3.5% had portable ventilator. Majority (75%) lacked the essential equipment as per AHA/national guidelines. Mean knowledge score of participants was 2.85 ± 4.10 and 62.25% and they had poor knowledge regarding CPR and only 12.25% had good/very good knowledge. Conclusion There is a lack of knowledge regarding CPR among ambulance staff and there is lack of equipped status of ambulances. They were not fully aware regarding the use of equipment, which were available in the ambulance in emergency. So, there is a need to train them about CPR as well utilization of ambulance equipment.
