How to use antistreptolysin O titre

Abstract: Group A streptococcus (GAS) is the cause of a wide range of acute suppurative and, following a latent period, non-suppurative diseases such as rheumatic fever and poststreptococcal glomerulonephritis. Diagnosis of the latter group requires evidence of preceding GAS infection. The bacteria produce a range of extracellular antigens, including streptolysin O, which induce an antibody response in the host. A rise in antistreptolysin O titre (ASOT) is indicative of preceding GAS infection. In clinical practice, oft… Show more

“…The exacerbation of symptoms must be associated with the infection of group A Streptococci, which needs to be confirmed with the positive swab from the throat and/or increased titers of antistreptolysin-O (ASO) or anti-DNase B [56,57]. In the case of PANDAS, GAS infection usually occurs without macroscopic changes in the throat and symptoms of pharyngitis.…”

“…An increase in ASO titer can be used as an indicator of previous GAS infection. In clinical practice, the timing of a GAS infection is unknown with a single ASO measurement (22). However, as in our study, in patients with progressive findings under standard treatment for soft tissue infections, considering the invasive GAS, clindamycin may be added to the treatment of toxin suppressive effects.…”

“…Also, ASO titer does not always indicate a recent streptococcal infection. Therefore, an increase in an ASO titer is indicative of a preceding group A Streptococcus infection, so sequential measurements are recommended [17-19]. We attempted to obtain 2 sequential measurements, but only a single sampling was performed in some patients.…”
