Hydrogen Sulfide Poisoning: Management and Complications

Abstract: Hydrogen sulfide poisoning is a rare but devastating injury. Four of 8 cases of toxic inhalation of hydrogen sulfide are presented with special attention to the life-threatening complications. Four patients died at the scene of the accident and the remaining 4 had been hospitalized in the Intensive Care Unit (ICU). One patient died in the hospital from cardiogenic pulmonary edema and circulatory failure (pulmonary wedge pressure > 30 mmHg) and 3 patients left the hospital in good health although they presented… Show more

“…Massive myocardial necrosis after exposure to hydrogen sulfide (at >500 ppm for more than 40 min) confirmed through postmortem autopsy 9) and two victims without autopsy, one of whom died following massive cardiogenic pulmonary edema and the other died of acute cardiac infarction two months after exposure to the gas has been described previously 10) . Later two victims also presented abnormal ECG with elevated ST segment in the left precordial leads 10) . One surviving case has shown abnormal ECG such as a posterolateral myocardial ischemic changes, which had returned to normal gradually 11) .…”

“…Os casos de intoxicação por H 2 S são mais acentuados principalmente no setor industrial. Segundo Gregorakos et al (1995), as autópsias realizadas em funcionários que morreram em um acidente envolvendo vazamento de sulfeto de hidrogênio revelaram que os corpos possuíam edemas pulmonares, miocardite, hemorragia na mucosa gástrica, além do cérebro e da parte posterior do intestino apresentar a cor esverdeada e alguns casos de ataque cardíaco.…”

“…The respiratory system, the circulatory system and the nervous system may be severely affected by H 2 S exposures [1,2]. Manifestations of cardiac diseases induced by H 2 S poisoning have been well demonstrated, including angina attack, myocardial infarction, arrhythmias and heart failure [3,4]. The prognosis is extremely poor and the best therapy has not yet been determined.…”
